基于倾向性评分匹配的早产儿暂时性甲状腺功能异常对生后早期发育的影响研究OA
Study on the impact of temporary thyroid dysfunction in preterm infants based on propensity score matching on early postnatal development
目的 探讨暂时性甲状腺功能异常对早产儿早期发育的影响.方法 收集2024年1月至12月晋江市医院收治的163例早产儿资料.根据生后2周内甲状腺功能是否异常将其分为异常组(n=32)和对照组(n=131).为控制基线混杂,采用倾向性评分匹配按1∶1为异常组匹配32例基线特征相似的对照组.比较匹配前后两组早产儿生后42d的体格发育及神经心理发育差异.神经心理发育采用0~6岁儿童智能发育筛查测验(development screen test,DST)和儿童神经心理行为检查量表(以下简称"儿心量表")进行评估.结果 匹配前,异常组早产儿的体质量、身长、头围Z值均显著低于对照组,且DST与儿心量表的异常率均显著高于对照组(P<0.001).匹配后,异常组早产儿的体质量、身长及头围Z值均显著低于对照组,儿心量表异常率显著高于对照组(P<0.05).多元回归显示甲状腺功能指标异常是体质量Z值降低和儿心量表异常风险增高的独立相关因素(P<0.05).结论 在控制混杂因素后,暂时性甲状腺功能异常仍是早产儿生后42d体格发育及神经心理发育落后的独立相关因素,提示应将其视为一个关键的临床预警信号,加强对此类高危儿的早期发育监测与干预.
Objective To explore the influence of temporary thyroid dysfunction on the early development of premature infants.Methods The data of 163 premature infants admitted to Jinjiang Municipal Hospital from January to December 2024 were collected.According to whether thyroid function was abnormal within 2 weeks after birth,they were divided into abnormal group(n=32)and control group(n=131).To control baseline confounding,propensity score matching was used to match 32 cases with similar baseline characteristics in abnormal group at a ratio of 1:1.The differences in physical development and neuropsychological development of two groups of premature infants 42 days after birth before and after matching were compared.Neuropsychological development was evaluated using the development screen test(DST)for children aged 0-6 and the children neuropsychological and behavioral scale(CNBS).Results Before matching,the Z values of body weight,body length and head circumference of premature infants in abnormal group were significantly lower than those in control group,and the abnormal rates of DST and CNBS were significantly higher than those in control group(P<0.001).After matching,the Z values of body weight,body length and head circumference of premature infants in abnormal group were significantly lower than those in control group,and the abnormal rate of CNBS was significantly higher than that in control group(P<0.05).Multiple regression showed that abnormal thyroid function indicators were independent related factors for decreased body weight Z value and increased risk of abnormal CNBS(P<0.05).Conclusion After controlling for confounding factors,temporary thyroid dysfunction remains an independent related factor for the delayed physical and neuropsychological development of premature infants on 42 days after birth,suggesting that it should be regarded as a key clinical early warning signal,and early development monitoring and intervention for such high-risk infants should be strengthened.
