金属元素混合暴露与甲状腺癌发病风险的关联研究OA

Association between mixed metal element exposure and the risk of thyroid carcinoma:a case-control study

目的:探讨尿液中金属元素水平与甲状腺乳头状癌(PTC)发病风险的关系,识别关键风险因子,并评估其在混合暴露下的联合效应及交互作用.方法:采用1∶1匹配的病例对照研究设计,纳入189对新发PTC和性别、年龄匹配的健康对照者.采用电感耦合等离子体质谱仪检测尿液中金属元素的浓度.采用条件logistic回归评估单一和混合金属元素暴露对PTC风险的影响,应用最小绝对收缩和选择算子(LASSO)回归筛选关键金属元素,并采用加权分位数和(WQS)回归识别混合暴露中主要风险贡献金属元素,最终构建贝叶斯核函数回归(BKMR)模型评估金属元素混合暴露的整体效应及金属元素间交互作用.结果:多因素条件logistic回归分析显示,在校正金属元素混合暴露模型中,尿液镍、钼和钡浓度升高与甲状腺癌风险增加相关,OR(95%CI)分别为2.08(1.53~2.96)、2.78(1.68~4.77)和2.42(1.80~3.33);硒和铜与甲状腺癌风险呈负相关,OR(95%CI)分别为0.20(0.15~0.37)和0.31(0.16~0.60).LASSO回归显示铬、镍、硒、钼、铅、铁、铜、钡等八种金属元素为关键因子.WQS回归分析显示,正向约束模型中镍、钡和钼权重较大,负向约束模型中铜和硒权重较大,提示上述金属元素可能是混合暴露效应的主要风险贡献因子.BKMR模型显示,金属元素混合暴露与甲状腺癌风险之间存在非线性关系,且金属元素间可能存在潜在效应修饰.结论:镍、钼和钡暴露可能是甲状腺癌发病的风险因素,铜可能具有一定保护作用,硒与甲状腺癌风险呈倒U型关联.金属元素混合暴露效应具有非线性和浓度依赖性,且不同金属元素间可能存在交互效应.

Objective:To investigate the association between urinary metal element concentrations and the risk of papillary thyroid carcinoma(PTC),to identify key risk factors,and to evaluate the combined effects and interactions of metal element exposure.Methods:A 1∶1 matched case-control study was adopted,including 189 pairs of newly diagnosed PTC patients and healthy controls matched by age and sex.Urinary concentrations of metal elements were measured using inductively coupled plasma mass spectrometry.Conditional logistic regression was applied to evaluate the associations of individual and mixed metal element exposure with PTC risk.Least absolute shrinkage and selection operator(LASSO)regression was used to screen for key metal elements.Weighted quantile sum(WQS)regression was employed to identify the main contributors to the mixture effect.Finally,a Bayesian kernel machine regression(BKMR)model was constructed to assess the overall effect of mixed metal element exposure and potential interactions among metal elements.Results:Multivariable conditional logistic regression showed that,in the model adjusted for multiple metal elements,elevated urinary concentra-tions of nickel,molybdenum,and barium were associated with an increased risk of thyroid carcinoma,with ORs(95%CIs)of 2.08(1.53-2.96),2.78(1.68-4.77),and 2.42(1.80-3.33),respectively;selenium and copper were negatively associated with thyroid carcinoma risk,with ORs(95%CIs)of 0.20(0.15-0.37)and 0.31(0.16-0.60),respectively.LASSO regression identified eight metal elements as key predictors:chromium,nickel,selenium,molybdenum,lead,iron,copper,and barium.WQS regression showed that nickel,barium,and molybdenum had relatively high weights in the positive constraint model,while copper and selenium had relatively high weights in the negative constraint model,suggesting that these metal elements may be major contributors to the overall mixture effect.The BKMR model indicated a nonlinear association between mixed metal element exposure and thyroid carcinoma risk,with potential effect modification among metal elements.Conclusions:Nickel,molybdenum,and barium exposure may be risk factors for thyroid carcinoma,copper may have a protective effect,while association between selenium and thyroid carcinoma risk showed an inverted U-shaped pattern.The effect of mixed metal element exposure was characterized by non-linearity and concentration dependence,with potential interactions among different metal elements.
