目的 探讨高频彩色多普勒超声在膝关节韧带损伤评估中的应用价值.方法 回顾性分析2023年3月至2025年3月联勤保障部队第九八二医院收治的接受高频彩色多普勒超声检查的152例疑似膝关节韧带损伤患者的临床资料,以关节镜手术为金标准,将患者分为韧损(膝关节韧带损伤)组(79例)与非韧损(半月板损伤、关节软骨损伤)组(73例).比较两组患者入院时韧带厚度、韧带横截面积、滑膜厚度、滑膜血流分级,并采用受试者工作特征(ROC)曲线评估上述超声参数对膝关节韧带损伤的诊断效能.根据Lysholm膝关节评分量表将韧损组患者分为轻中度(≥65分)组(48例)与重度(<65分)组(31例),比较两组患者入院时上述超声参数,并采用ROC曲线评估上述超声参数对膝关节韧带损伤严重程度的评估效能.结果 152例疑似膝关节韧带损伤患者中,同时接受高频彩色多普勒超声检查和MRI检查的患者123例.高频彩色多普勒超声与关节镜一致性的Kappa值为0.707,MRI与关节镜一致性的Kappa值为0.837.入院时,韧损组韧带厚度、韧带横截面积、滑膜厚度均显著高于非韧损组(P<0.05);韧损组与非韧损组滑膜血流分级比较,差异有统计学意义(P<0.05),韧损组滑膜血流0级、Ⅰ级比例低于非韧损组,Ⅱ级、Ⅲ级比例高于非韧损组;ROC曲线评估结果显示,韧带厚度、韧带横截面积、滑膜厚度、滑膜血流分级对膝关节韧带损伤诊断效能的AUC分别为0.823、0.829、0.789、0.752.轻中度组韧带厚度、韧带横截面积、滑膜厚度均显著低于重度组(P<0.05);轻中度组与重度组滑膜血流分级比较,差异有统计学意义(P<0.05),轻中度组滑膜血流0级、Ⅰ级比例高于重度组,Ⅲ级比例低于重度组;ROC曲线评估结果显示,韧带厚度、韧带横截面积、滑膜厚度、滑膜血流分级对膝关节韧带损伤严重程度的评估效能的AUC分别为0.767、0.749、0.760、0.743.结论 高频彩色多普勒超声参数对膝关节韧带损伤具有一定的诊断效能,且有助于判断膝关节韧带损伤严重程度.

Objective To explore the application value of high-frequency color Doppler ultrasound in the assessment of knee ligament injury.Methods The clinical data of 152 patients with suspected knee ligament injury who were admitted to the 982nd Hospital of the Joint Logistics Support Force and received high-frequency color Doppler ultrasound examination from March 2023 to March 2025 were retrospectively analyzed.Taking arthroscopic surgery as the gold standard,the patients were divided into ligament injury group(knee ligament injury)(n=79)and non-ligament injury group(meniscal injury,articular cartilage injury)(n=73).The differences in ligament thickness,ligament cross-sectional area,synovial thickness and synovial blood flow grading between the two groups of patients at admission were compared.ROC curve was used to evaluate the diagnostic efficiency of the above-mentioned ultrasonic parameters for knee ligament injury.The patients in the ligament injury group were classified into mild-to-moderate group(≥65 points)(n=48)and severe group(<65 points)(n=31)according to the Lysholm Knee Scoring Scale.The differences in the above-mentioned ultrasonic parameters at admission in the two subgroups were analyzed.ROC curve was used to evaluate the assessment efficiency of the above-mentioned indicators on the severity of knee ligament injury.Results Among the 152 patients with suspected knee ligament injury who were examined in this study,123 patients underwent high-frequency color Doppler ultrasound and MRI.The Kappa value for consistency between high-frequency color Doppler ultrasound and arthroscopy was 0.707,and the Kappa value for consistency between MRI and arthroscopy was 0.837.At admission,the ligament thickness,ligament cross-sectional area and synovial thickness in the knee ligament injury group were significantly higher than those in the non-ligament injury group(P<0.05).There was a statistically significant difference in synovial blood flow grading between the ligament injury group and the non-ligament injury group(P<0.05).The proportion of grade 0 and grade I synovial blood flow in the ligament injury group was lower than that in the non-ligament injury group,while the proportion of grade Ⅱ and grade Ⅲ was higher.ROC curve results showed that the areas under the curves for diagnosing knee ligament injury based on ligament thickness,ligament cross-sectional area,synovial thickness and synovial blood flow grading were 0.823,0.829,0.789 and 0.752 respectively.At admission,the ligament thickness,ligament cross-sectional area and synovial thickness in the mild-to-moderate group were significantly lower than those in the severe group(P<0.05).there was a statistically significant difference in synovial blood flow grading between the mild-to-moderate group and the severe group(P<0.05).The proportion of grade 0 and grade Ⅰ synovial blood flow in the mild-to-moderate group was higher than that in the severe group,while the proportion of grade Ⅲ was lower.ROC curve results showed that the areas under the curves for assessing the severity of knee ligament injury based on ligament thickness,ligament cross-sectional area,synovial thickness,and synovial blood flow grading were 0.767,0.749,0.760 and 0.743 respectively.Conclusion High-frequency color Doppler ultrasound parameters have certain diagnostic efficiency on knee ligament injury and are helpful in determining the severity of knee ligament injury.
