上海宠物源产ESBLs肺炎克雷伯菌分子特征研究OA
Study on the Molecular Characterization of Extended-Spectrum Beta-Lactamases-Producing Klebsiella pneumoniae of Pets in Shanghai
为探究上海地区宠物源产超广谱β-内酰胺酶(ESBLs)肺炎克雷伯菌的耐药表型和分子特征,以51 株犬猫源肺炎克雷伯菌为研究对象,采用微量肉汤稀释法测定ESBLs 表型及产ESBLs 肺炎克雷伯菌药物敏感性.选取菌株进行全基因组测序,分析耐药基因、毒力因子、ST 型和系统发育关系.结果显示,产ESBLs 肺炎克雷伯菌分离率为35.3%,且普遍存在多重耐药性(94.4%).对氨苄西林耐药率最高(100%),其次为庆大霉素和头孢噻呋(均为94.4%);对黏菌素耐药率最低(11.1%).菌株广泛携带SHV 型β-内酰胺类耐药基因(100%)、CTX-M-15 基因(40.0%)等.毒力因子以菌毛和铁载体为主,同时还检测到3 个高毒力生物标志基因.ST15 和 ST307 为优势克隆(各20%),并携带CTX-M-15、OXA-1 和SHV-28 基因组合.本研究表明上海地区宠物源产 ESBLs 肺炎克雷伯菌耐药形势严峻,亟需加强兽医临床用药监管并将宠物源肺炎克雷伯菌纳入耐药监测体系.
To investigate the resistance phenotypes and molecular characteristics of pet-derived extended-spectrum beta-lactamases(ESBLs)-producing Klebsiella pneumoniae in Shanghai,51 K.pneumoniae isolates from dogs and cats were studied.The micro broth dilution method was used to determine the ESBLs phenotype and the antimicrobial susceptibility of ESBLs-producing K.pneumoniae.Selected strains for whole-genome sequencing to analyse antibiotic resistance genes,virulence factors,ST types,and phylogenetic relationships.The results showed that the isolation rate of ESBLs-producing K.pneumoniae was 35.3%,and these strains were predominantly multidrug resistance(94.4%).The highest resistance rates were observed against Ampicillin(100%),followed by Gentamicin and Ceftiofur(both 94.4%),while the resistance rate to Colistin was the lowest(11.1%).The strains extensively harboured SHV-type β-lactamase resistance genes(100%)and CTX-M-15 genes(40.0%),among others.The virulence factors primarily identified were fimbriae and siderophores,and three high-virulence biomarker genes were detected.ST15 and ST307 were identified as the most prevalent clonal lineages(each accounting for 20%of isolates)and were found to harbour the CTX-M-15,OXA-1,and SHV-28 gene combinations.This study indicates that the antimicrobial resistance situation among ESBLs-producing K.pneumoniae isolated from pets in Shanghai is severe.There is an urgent need to strengthen oversight of veterinary drug use and incorporate pet-derived K.pneumoniae into antimicrobial resistance surveillance systems.
