目的 探讨MRI联合彩超血管测量在突发性耳聋(sudden sensorineural hearing loss,SSNHL)患者临床评估中的作用.方法 回顾性纳入2021年11月至2023年 12 月西南医科大学附属医院耳鼻咽喉头颈外科收治的268例SSNHL住院患者为研究对象,分析患者小脑前下动脉(anterior inferior cerebellar artery,AICA)血管袢Kazawa分型及彩色多普勒超声测量颈总动脉内中膜厚度(intima-media thickness of the common carotid artery,CCA-IMT),及SSNHL患者CCA-IMT和AICA血管袢Kazawa分型与SSNHL临床分型、听力损失程度、预后、血液学指标、年龄等相关性.结果 不同听力损失程度组SSNHL患者CCA-IMT比较,差异有统计学意义(P<0.05),不同听力改善平均阈值组AICA血管袢Kazawa分型比较,差异有统计学意义(P<0.05).CCA-IMT与SSNHL临床分型、预后、不同听力改善阈值无相关性(P>0.05).AICA血管袢Kazawa分型与SSNHL听力损失程度、临床分型、预后无相关性(P>0.05).AICA血管袢Kazawa分型和CCA-IMT无相关性(P>0.05).不同CCA-IMT组高血压病、糖尿病和耳鸣、性别、年龄、红细胞体积分布宽度-血清白蛋白比值分布比较,差异有统计学意义(P<0.05),眩晕分布比较,差异无统计学意义(P>0.05);不同AICA血管袢Kazawa分型组高血压病、糖尿病、耳鸣、眩晕分布比较,差异无统计学意义(P>0.05).结论 MRI联合彩超血管测量有助于判断SSNHL患者听力损失程度与听力改善效果.

Objective To investigate the value of combined magnetic resonance imaging(MRI)and color doppler ultrasound vascular measurement in clinical evaluation of patients with sudden sensorineural hearing loss(SSNHL).Methods Data from patients with SSNHL treated in our hospital(n=268)were retrospectively analyzed.The relationships between Kazawa type of the anterior inferior cerebellar artery(AICA)on MRI and intima-media thickness of the common carotid artery(CCA-IMT)measured by color doppler ultrasound,and SSNHL classification,degree of hearing loss,prognosis,hematological indicators and patients age were analyzed.Results CCA-IMT varied with the degree of hearing loss(P<0.05),and Kazawa type of AICA varied with hearing improvement(P<0.05).CCA-IMT and Kazawa type of AICA showed no correlation to SSNHL type,prognosis and hearing improvement(P>0.05).Kazawa type of AICA also showed no significant to degree of hearing loss(P>0.05).CCA-IMT and Kazawa type of AICA demonstrated no correlation to each other(P>0.05),although CCA-IMT was correlated to hypertension,diabetes mellitus,tinnitus,age,gender and red cell distribution width-to-albumin ratio(RAR)(P<0.05),but not to vertigo(P>0.05),whereas Kazawa type of AICA was correlated to none of these(P>0.05).Conclusions Combined MRI and color Doppler ultrasound vascular measurement can help assess the severity of hearing loss and predict hearing improvement outcomes in patients with SSNHL.
