Effects of postural changes on morphology of pharyngeal orifice in patients with patulous Eustachian tube

目的 探讨体位变化对咽鼓管异常开放患者咽鼓管咽口形态的影响,为临床诊断与治疗提供依据.方法 回顾性分析2020年6月至2024年12月解放军总医院耳鼻咽喉头颈外科收治的42例咽鼓管异常开放症患者咽鼓管咽口图像(其中咽鼓管异常开放且伴有鼓膜随呼吸扇动30例),纳入对照组20例,构建咽鼓管咽口图像诊断模型,并分别对坐位和卧位下的咽鼓管咽口形态进行综合评分.结果 构建咽鼓管咽口图像诊断模型识别准确率达到95%,卧位咽口评分显著低于坐位评分(中位数差异=-1.000,P<0.001),Spearman相关分析证实配对有效性(rs=0.564,P<0.001).两种体位下的黏膜状况及动态功能评分无显著差异(P>0.05).结论 本研究构建基于深度学习的咽鼓管异常开放图像诊断模型,可精准识别咽鼓管咽口形态,辅助疾病诊断;对鼻内镜下咽鼓管咽口图像分析发现,体位变化对咽鼓管咽口形态影响显著,且卧位开放程度小于坐位.

Objective To investigate the impact of postural changes on morphology of the pharyngeal orifice of the Eustachian tube in patients with patulous Eustachian tube(PET),to provide evidence for clinical diagnosis and treatment.Methods Imaging data of the Eustachian tube orifice from 42 PET patients admitted to our hospital between June 2020 and December 2024 were retrospectively analyzed.Among these patients,30 presented with tympanic membrane flutter synchronous with respiration.A control group(n=20)was included for comparison.A diagnostic model for Eustachian tube orifice images was constructed.All patients underwent nasopharyngoscopy in both sitting and lying positions,and comprehensive morphology scores of the Eustachian tube orifice were assigned separately for each posture.Results The diagnostic model for Eustachian tube orifice images achieved a recognition accuracy of 95%.The pharyngeal orifice score in the lying position was significantly lower than that in the sitting position(median difference=-1.000,P<0.001).Spearman correlation analysis confirmed the validity of the paired comparisons(rs=0.564,P<0.001).No significant differences were found in the scores for mucosal condition and dynamic function between the two postures(P>0.05).Conclusions The deep learning-based image diagnostic model for PET constructed in this study can accurately identify the morphology of the Eustachian tube orifice,providing a tool for investigation into the effect of posture.The scoring system refined based on observed morphological changes revealed that postural changes significantly affect the morphology of the Eustachian tube orifice,with the degree of opening being smaller in the lying position compared to the sitting position.No statistically significant differences were found in the scores for mucosal condition and dynamic function between the sitting and lying positions in this study.
