免疫检查点抑制剂(ICIs)是肿瘤免疫治疗领域的突破性成果之一,但也带来了一系列免疫相关不良反应.其中,消化系统毒性占有相当大的比例.ICIs相关消化系统毒性与风湿免疫性疾病中的自身免疫性肝病、炎症性肠病等在发病机制、临床表现及治疗方面具有很高的相似性."卫气内伐"理论最早出自《黄帝内经》,后成为指导风湿免疫性疾病治疗的重要学术思想之一.借鉴"卫气内伐"理论分析ICIs相关消化系统毒性,发现其与卫气升降出入失常或营气衰少相关.因此,宣肺洁肠以恢复卫之出入,祛瘀除湿以通畅卫道,疏肝理气以恢复卫之升降,补气养血以充润营气,可使卫气恢复正常的生理功能,达到治疗ICIs相关消化系统毒性的目的.

Immune checkpoint inhibitors(ICIs)are one of the breakthrough achievements in the field of tumor immunotherapy,but they also bring a series of immune related adverse reactions.Among them,digestive system toxicity accounts for a considerable proportion.ICIs related gastrointestinal toxicity shares high similarities with autoimmune hepatitis,inflammatory bowel disease,and other rheumatic and autoimmune diseases in terms of pathogenesis,clinical manifestations,and treatment.The theory of"defensive Qi internal logging"originated from the Inner Canon of Huangdi and later became one of the important academic ideas for rheumatic and immune diseases.Drawing on the theory of"defensive Qi internal logging"to analyze the digestive system toxicity of ICIs,it was found that they are related to abnormal ascending and descending,exiting and entering of defensive Qi or insufficient energy of the nutrient Qi.Therefore,diffusing the lung and clearing the intestine to restore the ascending and descending of the defensive Qi,eliminating blood stasis and removing dampness to dredge the channel of defensive Qi movement,soothing liver and regulating Qi to restore the exiting and entering of defensive Qi,replenishing Qi and nourishing blood to nourish the nutrient Qi can restore the normal physiological function of the defensive Qi and achieve the goal of treating ICIs related digestive system toxicity.
