基于"脾主为卫"探讨缺血性脑卒中脑-脾轴免疫稳态重建及中医药防治机制OA

Ischemic stroke(IS)is an acute ischemic-hypoxic injury of brain tissue caused by cerebrovascular obstruction,which belongs to the category of stroke in the context of traditional Chinese medicine(TCM).Studies have shown that the homeostatic imbalance between central neuroinflammation and peripheral immune response is a key pathological mechanism affecting the prognosis of IS.The spleen,as an important peripheral immune organ,regulates the migration of immune cells and the release of cytokines through the neural-immune regulatory network of the brain-spleen axis,thus participating in the reconstruction of immune homeostasis after IS.The theory of"spleen governing defense"originates from the Inner Canon of Huangdi,which emphasizes the importance of the spleen in the body's defense and the maintenance of the stability of the internal environment,and is highly compatible with the modern viewpoint of immunology.By improving spleen function,inhibiting spleen atrophy and regulating immune imbalance,the method of transporting the spleen to strengthen the defensive Qi can reduce neuroinflammatory reactions after IS and promote the repair of brain injury.Therefore,based on the theory of"spleen governing defense"and the modern medical research on the brain-spleen axis,this paper systematically discusses the dynamic regulation of the spleen in IS and the prevention and treatment mechanism of TCM,with the aim of providing a new theoretical basis and a targeted intervention strategy for the synergistic treatment of IS in both Chinese and Western medicine.
