耳穴三联疗法联合中药熏洗坐浴对湿热下注型低位单纯性肛瘘术后的疗效OA
Efficacy of auricular point triple therapy combined with traditional Chinese medicine fumigation and sitting bath on syndrome of downward flow of damp-heat dampness of anal fistula cases after surgery
目的:探讨耳穴三联疗法联合中药熏洗坐浴对湿热下注型低位单纯性肛瘘(LSAF)术后患者的疗效.方法:选取2023年1月-2025年1月在南京市中医院行LSAF手术治疗的患者122例,随机分成对照组和联合组两组,每组各61例.两组均行LSAF,术后给予抗感染、止血等常规药物.对照组于术后2d行耳穴三联疗法.联合组耳穴三联疗法同对照组,并于术后2d采取院内制剂"消肿洗剂"中药熏洗坐浴.治疗14 d后,比较两组肛周疼痛视觉模拟量表(VAS)评分、肛缘水肿评分、疼痛与水肿的消失时间、创面愈合时间、血清血管内皮生长因子(VEGF)水平及治疗效果.结果:术后7d、14d,两组VAS评分及肛缘水肿评分均明显降低,并且联合组低于对照组,差异有统计学意义(P＜0.05).与对照组比较,联合组的疼痛和水肿消失时间明显缩短,差异有统计学意义(P＜0.05).联合组创面愈合时间(14.33±3.16)d明显短于对照组(16.93±3.44)d,差异有统计学意义(P＜0.05).术后7d、14d,两组血清VEGF水平显著增加,并且联合组高于对照组,差异有统计学意义(P＜0.05).联合组的总有效率(93.44％)高于对照组(78.69％),差异有统计学意义(P＜0.05).结论:耳穴三联疗法联合中药熏洗坐浴能明显减轻湿热下注型LSAF手术后疼痛,改善肛缘水肿,促进创面愈合,疗效明显,并上调血清VEGF水平.
Objective To analyze the efficacy of auricular point triple therapy combined with traditional Chinese medicine fumigation and sitting bath on syndrome of downward flow of damp-heat dampness of anal fistula cases after low simple anal fistula(LSAF)surgery.Methods A total of 122 patients who underwent LSAF surgery in Nanjing hospital of Chinese medicine from January of 2023 to January of 2025 were selected and randomly divided into two groups with 61 patients in each group.Participants in both groups received LSAF surgery and were given anti-infection,hemostasis,and other conventional drugs after surgery.The control group received auricular point triple therapy on the 2nd day after operation.The combined group was treated with three auricle points as the control group and self-made preparation"detumescence lotion"traditional Chinese medicine fumigation and sitting-bath on the 2nd day after operation.The two groups were observed for 14 days.The perianal pain visual analogue scale(VAS)and anal edema score,the time for pain and edema to disappear,wound healing time,treatment effect and serum level of vascular endothelial growth factor(VEGF)were compared between the two groups.Results On day 7 and 14 after operation,VAS scores and scores of anal edema in both groups were evidently reduced,and the reduction was more evident in the combined group(P＜0.05).Compared with the control group,the time for pain and edema to disappear in the combined group was evidently reduced(P＜0.05).The wound healing time in combination group(14.33±3.16)d was evidently shorter than that in control group(16.93±3.44)d(P＜0.05).Serum level of VEGF in the two groups were evidently increased at 7 and 14 days after operation,and the cases in the combined group were evidently increased(P＜0.05).The total effective rate of the combined group(93.44％)was evidently higher than control group(78.69％)(P＜0.05).Conclusion Auricular point triple therapy combined with Chinese herbal fumigation and sitting-bath can evidently reduce pain after LSAF surgery,improve anal edema,promote wound healing,has an evident efficacy,and increase serum level of VEGF.
