嘌呤能系统在癫痫机制中的作用及诊疗研究进展OA
Advances in the role of the purinergic system in epileptic mechanisms and its diagnosis and treatment
癫痫是一种常见的慢性神经系统疾病,目前临床诊断主要依赖病史与脑电图,存在一定局限性.现有治疗以控制发作为主,难以阻断疾病进展,且约30%的患者为药物难治性癫痫,预后不佳.近年来研究发现,嘌呤能系统在调控神经元兴奋性与神经炎症中发挥重要作用,ATP-P2X7受体信号通路与腺苷-A1受体信号通路的失衡参与癫痫的发生发展.该系统相关分子在癫痫的多维度诊断中展现出应用价值,针对该系统的靶向干预策略也具备广阔前景,但临床转化仍面临挑战.本文系统综述嘌呤能系统在癫痫中的作用机制,重点评估其在临床诊断与治疗中的应用价值,为未来实现癫痫的精准诊疗提供理论依据与策略.
Epilepsy is a common chronic neurological disease.Current clinical diagnosis mainly relies on medical history and electroencephalography,which has certain limitations.Existing treatments mainly focus on seizure control but are unable to halt disease progression,and approximately 30%of patients have drug-resistant epilepsy with poor prognosis.In recent years,studies have found that the purinergic system plays an important role in regulating neuronal excitability and neuroinflammation,and the imbalance between the ATP-P2X7R signaling pathway and the adenosine-A1R signaling pathway is involved in the occurrence and development of epilepsy.Molecules related to this system have shown application value in the multi-dimensional diagnosis of epilepsy,and targeted intervention strategies for this system also have broad prospects,but clinical translation still faces challenges.This article systematically reviews the role of the purinergic system in epilepsy,focuses on evaluating its application value in clinical diagnosis and treatment,and provides a theoretical basis and strategies for achieving precision diagnosis and treatment of epilepsy.
