前庭性偏头痛无创神经调控治疗的研究进展OA
Research progress on non-invasive neuromodulation therapy for vestibular migraine
前庭性偏头痛是偏头痛与前庭功能障碍的交叉性疾病,传统治疗主要基于标准偏头痛药物治疗,但药物不良反应常导致患者依从性差.随着神经调控技术不断发展,非侵入性神经调控已成为前庭性偏头痛的潜在治疗策略.本文系统分析国内外关于各类非侵入性神经调控技术在前庭性偏头痛管理中的最新研究进展,重点讨论重复经颅磁刺激、经颅直流电刺激、非侵入性迷走神经刺激和经皮三叉神经刺激4种主要疗法.与传统药物治疗相比,这些非侵入性技术具有安全性高、全身副作用少、患者依从性好等优势.临床证据表明,这些神经调控方法可有效缓解前庭性偏头痛患者的前庭症状(如眩晕、姿势不稳)和头痛发作,其机制可能涉及调节皮层兴奋性、脑干核团活动及三叉-前庭通路连接.本文旨在为临床医生和研究人员提供前庭性偏头痛非侵入性治疗的参考.
Vestibular migraine(VM),as a cross-disciplinary disorder of migraine and vestibular dysfunction,has traditionally relied on standard migraine medications,but adverse drug reactions often lead to poor patient compliance.With the continuous advancement of neuromodulation technologies,non-invasive neuromodulation has gradually emerged as a promising therapeutic approach for VM.This review systematically synthesizes and analyzes the latest domestic and international research progress on the application of various non-invasive neuromodulation techniques in the management of vestibular migraine.Specifically,we discuss the mechanisms,clinical evidence,and therapeutic potential of four main modalities:repetitive transcranial magnetic stimulation(rTMS),transcranial direct current stimulation(tDCS),non-invasive vagus nerve stimulation(nVNS),and external trigeminal nerve stimulation(eTNS).Compared with conventional pharmacological interventions,these non-invasive techniques offer distinct advantages,including a favorable safety profile,minimal systemic side effects,and improved patient compliance.Accumulating evidence from clinical studies suggests that these neuromodulation approaches can effectively alleviate both vestibular symptoms(such as vertigo and postural instability)and headache episodes in patients with VM,potentially by modulating cortical excitability,brainstem nuclei activity,and the trigeminal-vestibular pathways connectivity.This review aims to provide a reference for clinicians and researchers interested in the non-invasive management of VM.
