慢性病共病的管理困境与对策研究OA

At present,chronic disease comorbidity has become a major public health challenge,with its prevalence continuously rising and the disease burden increasingly heavy.Although there have been studies explo-ring chronic disease comorbidity management from multiple aspects,most of them are limited to a single entity or a specific step,lacking a systematic analysis from a collaborative management perspective.Based on the review of the management practices of chronic disease comorbidity,this paper constructs a tripartite analysis framework of government,healthcare institutions and patients,and analyzes in-depth the core problems currently faced:at the government level,there is a lack of diagnostic guidelines and insufficient knowledge popularization;at the health-care institution level,there is fragmentation in resource allocation and mismatch in service models;at the patient level,there is a lack of self-management ability and heavy psychological and social burdens.In response to these problems,it proposes systematic countermeasures such as developing guidelines for the diagnosis and treatment of chronic disease comorbidity,optimizing integrated medical resource allocation,and building a networked social support system,aiming to promote the transformation of chronic disease comorbidity management from a single dis-ease-oriented approach to multimorbidity co-management,from fragmented services to continuous care,thus provi-ding theoretical basis and practical paths for achieving systematic,continuous and individualized management of chronic disease comorbidity.
