创伤性脑损伤患者运动功能恢复影响因素的Meta分析OA
Influencing factors of motor function recovery in patients with traumatic brain injury:a Meta-analysis
目的 系统评价创伤性脑损伤(TBI)患者运动功能恢复的影响因素.方法 计算机检索PubMed、Web of Science、The Cochrane Library、Embase、中国知网、万方、维普、中国生物医学文献数据库,搜集关于TBI患者运动功能恢复影响因素的研究,检索时限均为建库至2025年9月30日.由2名研究者独立完成文献筛选、资料提取及纳入研究的偏倚风险评价后,采用RevMan 5.3软件进行Meta分析.结果 共纳入22项研究,其中队列研究18项、横断面研究4项,样本量为6 425例,涉及47项影响因素.Meta分析结果显示,年龄[OR=1.55,95%CI(1.17,2.05)]、年龄>50岁[OR=2.03,95%CI(1.20,3.43)]、格拉斯哥昏迷量表(GCS)评分[OR=0.28,95%CI(0.14,0.66)]、运动功能区损伤[OR=3.12,95%CI(1.74,5.60)]、脑疝[OR=1.99,95%CI(1.36,2.89)]、意识障碍时间>10 d[OR=2.33,95%CI(1.18,4.60)]、康复治疗介入时间>30 d[OR=2.46,95%CI(1.48,4.09)]、康复治疗时间[OR=1.14,95%CI(1.07,1.21)]及男性[OR=1.80,95%CI(1.21,2.67)]是TBI运动功能恢复的影响因素(P<0.05).结论 当前证据显示,年龄较大、年龄>50岁、GCS评分较低、运动功能区损伤、脑疝、意识障碍时间>10 d、康复治疗介入时间>30 d、康复治疗时间较长及男性是TBI患者运动功能恢复不良的危险因素.临床医护人员需重点关注上述危险因素并及时干预,提高TBI患者运动功能的康复效果.
Objective To systematically review the influencing factors of motor function recovery in patients with traumatic brain injury(TBI).Methods PubMed,Web of Science,The Cochrane Library,Embase,CNKI,WanFang Data,VIP,and SinoMed databases were electronically searched to collect studies on the influencing factors of motor function recovery in TBI patients from inception to September 30,2025.Two reviewers independently screened the literature,extracted data and assessed the risk of bias of the included studies.Meta-analysis was performed using RevMan 5.3 software.Results A total of 22 studies were included,including 18 cohort studies and 4 cross-sectional studies,with a sample size of 6,425 cases and 47 factors involved.The results of the Meta-analysis showed that age[OR=1.55,95%CI(1.17,2.05)],age>50 years[OR=2.03,95%CI(1.20,3.43)],Glasgow Coma Scale(GCS)score[OR=0.28,95%CI(0.14,0.66)],motor function area injury[OR=3.12,95%CI(1.74,5.60)],brain herniation[OR=1.99,95%CI(1.36,2.89)],duration of consciousness disorder>10 days[OR=2.33,95%CI(1.18,4.60)],rehabilitation intervention time>30 days[OR=2.46,95%CI(1.48,4.09)],rehabilitation treatment time[OR=1.14,95%CI(1.07,1.21)],and male gender[OR=1.80,95%CI(1.21,2.67)]were the influencing factors of motor function recovery in TBI patients(P<0.05).Conclusion Current evidence shows that advanced age,age>50 years,lower GCS score,motor function area injury,brain herniation,duration of consciousness disorder>10 days,rehabilitation intervention time>30 days,longer rehabilitation treatment time,and male gender are the risk factors for poor motor function recovery in TBI patients.Medical staff should pay close attention to these risk factors and intervene promptly to improve the rehabilitation effect of motor function in TBI patients.
