目的 探讨拟除虫菊酯暴露与肌肉减少症之间的关系.方法 采用横断面研究设计,基于2011-2016年美国国家健康与营养调查数据,筛选18~59岁成年人,通过双能X射线吸收测量法扫描技术计算四肢骨骼肌质量与体重指数(BMI)的比值判定肌肉减少症的患病情况,采用尿液中3-苯氧基苯甲酸(3-PBA)水平评估拟除虫菊酯暴露水平,通过加权Logistic回归和限制性立方样条(RCS)模型分析其与肌肉减少症的关联及其剂量-反应关系,并采用亚组分析和交互作用检验探讨结果的稳定性及BMI的修饰效应.结果 共纳入2 867名研究对象,肌肉减少症占比为7.57%.加权Logistic回归结果显示,调整协变量后,3-PBA暴露水平每增加1个对数单位,肌肉减少症风险增加21%[OR=1.21,95%CI(1.02,1.44),P=0.032];按ln(3-PBA)的四分位数分组,与Q1组相比,Q4组的肌肉减少症风险增加112%[OR=2.12,95%CI(1.22,3.69),P=0.009].RCS模型分析显示,尿3-PBA水平与肌肉减少症风险之间呈正向线性关联(总体P<0.001,非线性P=0.595),且当尿3-PBA水平超过0.57 μg/L后,风险上升趋势更为明显.亚组与交互作用分析显示,该关联在BMI≥30 kg/m2的肥胖个体中更强[OR=1.43,95%CI(1.19,1.71),趋势P<0.001],且BMI与3-PBA在肌肉减少症风险上具有交互作用[OR=1.59,95%CI(1.18,2.14),交互P=0.003].结论 拟除虫菊酯暴露与肌肉减少症风险之间呈正向关联,在肥胖人群中尤为明显.建议采取多层次防控措施,并需进一步研究以完善预防策略.

Objective To explore the relationship between pyrethroid exposure and sarcopenia.Methods Using a cross-sectional study design,the data were sourced from the U.S.National Health and Nutrition Examination Survey conducted from 2011 to 2016.Adults aged 18-59 years were selected as research subjects.Sarcopenia was assessed by calculating the ratio of skeletal muscle mass to body mass index(BMI)using dual-energy X-ray absorptiometry scanning technology.Pyrethroid exposure levels were evaluated based on urinary 3-phenoxybenzoic acid(3-PBA)concentrations.The association between these factors and the risk of sarcopenia,as well as their dose-response relationships,were analyzed using weighted Logistic regression and restricted cubic spline(RCS)models.Subgroup analyses and interaction tests were employed to examine the stability of the findings and the moderating effect of BMI.Results A total of 2,867 participants were included,with a prevalence of sarcopenia at 7.57%.Weighted Logistic regression analysis showed that,after adjusting for covariates,for every one-unit increase in log-transformed 3-PBA exposure level,the risk of sarcopenia increased by 21%[OR=1.21,95%CI(1.02,1.44),P=0.032];Group by quartile of ln(3-PBA),compared with the Q1 group,the Q4 group exhibited a 112%increased risk of sarcopenia[OR=2.12,95%CI(1.22,3.69),P=0.009].RCS analysis revealed a linear positive correlation between 3-PBA and the risk of sarcopenia(Poverall<0.001,Pnonlinearity=0.595),with the risk increase becoming more pronounced when urinary 3-PBA levels exceeded 0.57 μg/L.Subgroup and interaction analyses indicated that this association was stronger in obese individuals with a BMI≥30 kg/m2[OR=1.43,95%CI(1.19,1.71),Ptrend<0.001],and there was an interaction between BMI and 3-PBA on the risk of sarcopenia[OR=1.59,95%CI(1.18,2.14),Pinteraction=0.003].Conclusion There is a positive association between pyrethroid exposure and the risk of sarcopenia,which is particularly evident in obese populations.It is recommended to take multi-level prevention and control measures and further research is needed to improve prevention strategies.
