乌梅丸加减联合温针灸治疗子宫内膜异位症伴痛经的临床效果研究OA
Study on the Clinical Effect of Modified Wumei Pill Combined with Warm Acupuncture and Moxibustion in the Treatment of Endometriosis Complicated with Dysmenorrhea
目的 研究乌梅丸加减联合温针灸治疗子宫内膜异位症(EMT)伴痛经患者的临床效果.方法 选取我院2023年8月至2025年8月期间确诊为EMT伴痛经患者124例,采用随机数字表法将其分为对照组与研究组各62例.对照组单纯口服屈螺酮炔雌醇片,研究组在此基础上采用乌梅丸加减联合温针灸治疗.比较两组的临床疗效、疼痛与症状评分、生化指标[血清糖类抗原125(CA125)、前列腺素E2(PGE2)、前列腺素F2α(PGF2α)、肿瘤坏死因子α(TNF-α)及白细胞介素6(IL-6)]水平及不良反应发生情况.结果 两组的临床疗效评定等级分布具有明显差异(Z=-2.987,P=0.003);研究组的治疗总有效率为91.94％,明显高于对照组的77.42％(P＜0.05).治疗后,研究组的视觉模拟量表(VAS)、Cox痛经症状量表(CMSS)评分及血清CA125、PGE2、PGF2α、TNF-α、IL-6水平均显著低于对照组(P＜0.05).研究组的不良反应总发生率低于对照组(3.23％vs.12.90％,P＜0.05).结论 在口服屈螺酮炔雌醇片的基础上,采用乌梅丸加减联合温针灸治疗可进一步提升EMT伴痛经患者的临床效果,有效缓解患者的痛经症状,降低血清致痛前列腺素、促炎因子及CA125水平,同时减少西药相关不良反应,具有较高的临床推广价值.
Objective To investigate the clinical effect of modified Wumei Pill combined with warm acupuncture and moxibustion in the treatment of patients with endometriosis(EMT)complicated with dysmenorrhea.Methods A total of 124 patients diagnosed with EMT complicated with dysmenorrhea in our hospital from August 2023 to August 2025 were selected and divided into the control group and the study group by the random number table method,with 62 cases in each group.The control group was treated with oral Drospirenone and Ethinylestradiol Tablets alone,while the study group was treated with modified Wumei Pill combined with warm acupuncture and moxibustion on this basis.The clinical efficacy,pain and symptom scores,levels of biochemical indicators[serum carbohydrate antigen 125(CA125),prostaglandin E2(PGE2),prostaglandin F2α(PGF2α),tumor necrosis factor-α(TNF-α)and interleukin-6(IL-6)]and the occurrence of adverse reactions were compared between the two groups.Results There was a significant difference in the distribution of clinical efficacy grades between the two groups(Z=-2.987,P=0.003).The total effective rate of the study group was 91.94％,significantly higher than 77.42％of the control group(P＜0.05).After treatment,the Visual Analogue Scale(VAS)score,Cox Menstrual Symptom Scale(CMSS)score and the levels of serum CA125,PGE2,PGF2α,TNF-α and IL-6 in the study group were significantly lower than those in the control group(P＜0.05).The total incidence of adverse reactions in the study group was lower than that in the control group(3.23％vs.12.90％,P＜0.05).Conclusions On the basis of oral Drospirenone and Ethinylestradiol Tablets,modified Wumei Pill combined with warm acupuncture and moxibustion can further improve the clinical efficacy of patients with EMT complicated with dysmenorrhea,effectively relieve dysmenorrhea symptoms,reduce the levels of serum pain-causing prostaglandins,pro-inflammatory factors and CA125,and reduce western medicine-related adverse reactions,which has high clinical promotion value.
