非结核分枝杆菌高风险人群中医药干预疗效评估OA
Evaluation of efficacy of traditional Chinese medicine intervention in high-risk populations with nontuberculous mycobacteria
目的 建立非结核分枝杆菌(NTM)高风险人群量化评估体系,探讨中医药干预对NTM 高风险人群的临床疗效、肺功能和营养状态等身体状况的影响.方法 回顾性分析2024年8月—2025年7月符合纳入标准的NTM 高风险患者86例.根据患者是否联合中医治疗及干预分成观察组(43例)和对照组(43例).对照组给予西医常规抗非结核分枝杆菌标准化治疗,观察组在常规治疗基础上配合中医药干预(中药辨证论治联合艾灸、穴位贴敷综合干预),疗程12周.使用自制的NTM高风险人群评估量化评分表[包括6分钟步行测试(6-MWT)、肺功能、BMI、基础疾病、精神状态5个维度],比较两组治疗前后各项评分、实验室指标和症状改善情况.结果 治疗前两组6-MWT 评分、肺功能评分、基础疾病评分、抑郁评分及总分比较均差异无统计学意义(P>0.05);治疗 12 周后,观察组 6 分钟步行距离为(412.68±58.32)m,较对照组的(356.42±61.57)m明显提高,肺功能评分(1.82±0.76)分、基础疾病评分(1.12±0.91)分、抑郁评分(0.87±0.32)各项均低于对照组(P<0.01),观察组的血红蛋白、白蛋白水平和主要症状缓解率(咳嗽缓解率90.70%、咳痰缓解率86.05%、气促缓解率81.40%)高于对照组(P<0.05).观察组总有效率为88.37%,显著高于对照组的67.44%(P=0.019).结论 中医药干预可改善NTM高风险人群的营养状况、肺功能、运动耐力,减轻基础疾病负荷,可显著缓解患者抑郁情绪,临床疗效显著,可为NTM高风险人群的综合防治提供参考.
Objective To establish a quantitative evaluation system for high-risk populations with nontuberculous myco-bacteria(NTM)and to explore the impact of traditional Chinese medicine(TCM)intervention on clinical efficacy,pulmona-ry function,nutritional status and other physical conditions in this population.Methods A retrospective analysis was conducted on 86 high-risk NTM patients who met the inclusion criteria from August 2024 to July 2025.Based on whether they combined TCM treatment and intervention,they were divided into observation group(43 cases)and control group(43 cases).The control group were given standard Western medicine treatment for anti-NTM,while the observation group were given TCM intervention(comprehensive intervention of TCM syndrome differentiation and treatment combined with moxibus-tion and acupoint application)on the basis of conventional treatment for a course of 12 weeks.A self-made NTM high-risk population quantitative assessment scale(including 5 dimensions:6-minute walk test[6-MWT],pulmonary function,BMI,underlying diseases,and mental state)was used to compare various scores,laboratory indicators,and symptom improvement between the two groups before and after treatment.Results Before treatment,there were no statistically significant differ-ences in 6-MWT scores,pulmonary function scores,underlying disease scores,depression scores,and total scores between the two groups(P>0.05).After 12 weeks of treatment,the 6-minute walk distance in the observation group was(412.68±58.32)m,which was significantly increased compared with that([356.42±61.57]m)in the control group,while the pul-monary function score([1.82±0.76]points),underlying disease score([1.12±0.91]points),and depression score([0.87±0.32]points)in the observation group were all lower than those in the control group(P<0.01).Hemoglobin and albumin levels and major symptom remission rates(cough remission rate[90.70%],sputum remission rate[86.05%],and dyspnea remission rate[81.40%])in the observation group were all higher than those in the control group(P<0.05).The total effective rate in the observation group was 88.37%,which was significantly higher than that(67.44%)in the con-trol group(P=0.019).Conclusion TCM intervention can improve the nutritional status,pulmonary function,and exer-cise endurance of high-risk NTM population,reduce the burden of underlying diseases,and significantly alleviate patients'depressive emotions.The clinical efficacy is significant,which provides a reference for the comprehensive prevention and treatment of high-risk NTM populations.
