Study on ultrasonographic morphology and stiffness of the geniohyoid muscle in middle-aged and elderly adults

目的 探讨高频超声联合剪切波弹性成像(SWE)量化评估健康中老年人颏舌骨肌形态与硬度的应用价值,建立无创评估吞咽相关肌肉功能的影像学方法.方法 选取 2023 年 7 月至 2024 年 12 月招募的 111 名 45~80岁健康志愿者,其中男性 54 名,女性 57 名.采用高频超声测量颏舌骨肌最厚处前后径(D)与横截面积(CSA),采用SWE 技术测量其剪切波弹性杨氏模量值(E).通过组内相关系数(ICC)评价各参数的观察者内及观察者间测量重复性,采用 Pearson 相关分析与逐步多元线性回归分析探讨 D、CSA、E 与年龄、性别、体重指数(BMI)的关联.结果男性组颏舌骨肌 D[(0.61±0.08)cm vs(0.54±0.07)cm]、CSA[(2.06±0.48)cm2 vs(1.27±0.34)cm2]均大于女性组,差异有统计学意义(P 均<0.001);两组 E 值差异无统计学意义(P=0.088).D、CSA、E 的观察者内及观察者间ICC 均>0.9,提示测量高度一致性.相关性分析显示,D、CSA 与年龄呈负相关(r=-0.54、-0.59,P 均<0.001),与BMI 呈正相关(r=0.66、0.47,P 均<0.001);E 与 BMI 呈正相关(r=0.50,P<0.001).逐步多元线性回归分析显示,D、CSA 均与 BMI、性别独立相关(P 均<0.001),与年龄无独立关联;E 仅与 BMI 独立相关(P<0.001).结论 高频超声联合 SWE 可稳定、重复地量化中老年人颏舌骨肌的形态与硬度,形态学参数(D、CSA)与年龄、BMI 存在显著关联,硬度参数(E)与 BMI 存在显著正相关,有望为临床吞咽功能评估提供客观的影像学定量指标.

Objective To explore the application value of high-frequency ultrasound combined with shear-wave elastogra-phy(SWE)in quantitative evaluation of the morphology and stiffness of geniohyoid muscle in healthy middle-aged and elder-ly adults,so as to establish a non-invasive imaging method for assessing swallowing-related muscle function.Methods A total of 111 healthy volunteers aged 45 to 80 years were recruited from July 2023 to December 2024,including 54 males and 57 females.High-frequency ultrasound was used to measure the anterior-posterior diameter(D)and cross-sectional area(CSA)of geniohyoid muscle at its thickest point,and SWE technology was applied to measure its shear wave elastic Young's modulus(E).The intra-and inter-observer measurement repeatability of each parameter was evaluated by intraclass correlation coeffi-cient(ICC).Pearson correlation analysis and stepwise multiple linear regression analysis were performed to investigate the associations of D,CSA and E with age,gender and body mass index(BMI).Results The D([0.61±0.08]cm vs[0.54±0.07]cm)and CSA([2.06±0.48]cm2 vs[1.27±0.34]cm2)of geniohyoid muscle in the male group were significantly larger than those in the female group,and differences were statistically significant(all P<0.001).There was no statistically significant difference in E value between the two groups(P=0.088).The intra-and inter-observer ICCs of D,CSA and E were all greater than 0.9,indicating good measurement repeatability.Correlation analysis showed that D and CSA were nega-tively correlated with age(r=-0.54,-0.59,all P<0.001)and positively correlated with BMI(r=0.66,0.47,all P<0.001).E was positively correlated with BMI(r=0.50,P<0.001).Stepwise multiple linear regression analysis showed that both D and CSA were independently correlated with BMI and gender(all P<0.001),and had no independent associa-tion with age.E was only independently correlated with BMI(P<0.001).Conclusion High-frequency ultrasound com-bined with SWE enables stable and reproducible quantification of the morphology and stiffness of the geniohyoid muscle in middle-aged and elderly adults.Morphological parameters(D and CSA)are significantly associated with age and BMI,while the stiffness parameter(E)is significantly positively correlated with BMI,and this approach is expected to provide objective quantitative imaging indicators for clinical assessment of swallowing function.
