基于巨噬细胞极化及p38 MAPK信号通路探讨黄芪建中汤合小承气汤治疗术后肠梗阻的机制OA
Mechanism of Huangqi Jianzhong Decoction combined with Xiaochengqi Decoction in the treatment of postoperative ileus based on macrophage polarization and P38MAPK signal pathway
目的 探讨黄芪建中汤合小承气汤灌肠治疗术后肠梗阻的效应机制.方法 将24只 SPF 级雄性 Wistar 大鼠随机分为空白组、模型组、中药灌肠组和肥皂水灌肠组,每组6 只.除空白组外,其余组大鼠建立术后肠梗阻模型.术后24 h,中药灌肠组给予黄芪建中汤合小承气汤保留灌肠,肥皂水灌肠组给予肥皂水保留灌肠,均1 次/d,连续灌肠7 d.检测各组大鼠的肠道推进率,进行肠道 Butzner 评分,ELISA 法检测腹主动脉血中白细胞介素(IL)-6、肿瘤坏死因子-α(TNF-α)、IL-10、CD86、CD206 水平,Western blot 法检测小肠组织中 p38 丝裂原活化蛋白激酶(p38 MAPK)及磷酸化 p38 MAPK(p-p38 MAPK)蛋白表达情况.结果 与空白组比较,模型组大鼠肠道推进率明显降低(P<0.05),Butzner 评分及血清 IL-6、TNF-α、IL-10、CD86、CD206 水平和小肠组织中 p38 MAPK、p-p38 MAPK 蛋白相对表达量均明显升高(P 均<0.05).与模型组比较,中药灌肠组大鼠肠道推进率无明显变化(P>0.05),Butzner 评分及血清 IL-6、TNF-α、CD86、CD206 水平和小肠组织中 p38 MAPK、p-p38 MAPK 蛋白相对表达量均明显降低(P 均<0.05),血清 IL-10 水平明显升高(P<0.05);肥皂水灌肠组大鼠肠道推进率无明显变化(P>0.05),Butzner评分及血清 IL-6、TNF-α、IL-10、CD206 水平和小肠组织中 p38 MAPK、p-p38 MAPK 蛋白相对表达量均明显降低(P 均<0.05).且中药灌肠组 Butzner 评分及小肠组织中 p38 MAPK、p-p38 MAPK 蛋白相对表达量均明显低于肥皂水灌肠组(P 均<0.05),血清 IL-10、CD206 水平均明显高于肥皂水灌肠组(P 均<0.05).结论 黄芪建中汤合小承气汤可通过调控巨噬细胞极化及下游p38 MAPK 信号通路的激活,有效缓解术后肠梗阻大鼠的肠道炎症反应,促进胃肠功能恢复.
Objective It is to investigate the therapeutic mechanism of Huangqi Jianzhong Decoction combined with Xi-aochengqi Decoction in the treatment ofpostoperative ileus(POI).Methods Twenty-four SPF-grade male Wistar rats were randomly divided into blank control group,model group,herbal enema group and soap-water enema group,with 6 rats in each group.The rats of all groups except for the blank group were prepared to establish models of POI.In 24 hours after surgery,the herbal enema group was given Huangqi Jianzhong Decoction combined with Xiaochengqi Decoction via enema,the soap-water enema group was given soap-water via enema,all once daily,continuously treated fo 7 days.The intestinal transit rate of rats in each group was detected,their Butzner scores of intestines were assessed,the levels of interleukin(IL)-6,tumor necrosis factor-α(TNF-α),IL-10,CD86,and CD206 in abdominal aortic blood were measured by ELISA,and the protein expressions of p38 mitogen-activated protein kinase(p38 MAPK)and phosphorylated p38 MAPK(p-p38 MAPK)in small intestinal tissue were detected by Western blot method.Results Compared with the control group,the intestinal transit rate of rats in the model group was significantly reduced(P<0.05),while the Butzner scores and ser-um levels of IL-6,TNF-α,IL-10,CD86 and CD206,as well as the relative protein expressions of p38 MAPK and p-p38 MAPK in small intestinal tissue were significantly increased(all P<0.05).There was no significant difference in intesti-nal transit rate of rats among the model group,herbal enema group and soap-water enema group(all P>0.05),the Butzner score,serum levels of IL-6,TNF-α,CD86 and CD206,and the relative protein expressions of p38 MAPK and p-p38 MAPK in small intestinal tissue were all significantly lower(all P<0.05),while the serum level of IL-10 was signifi-cantly higher in the herbal enema group than those in the model group(P<0.05);the Butzner score,serum levels of IL-6,TNF-α and CD206,and the relative protein expressions of p38 MAPK and p-p38 MAPK in small intestinal tissue were all significantly lower in the soapy-water enema group than those in the model group(all P<0.05).Furthermore,the Butzner scores and the relative protein expressions of p38 MAPK and p-p38 MAPK in small intestinal tissue were signifi-cantly lower,while the serum levels of IL-10 and CD206 were significantly higher in the herbal enema group than those in the soap-water enema group(all P<0.05).Conclusion Huangqi Jianzhong Decoction combined with Xiaochengqi Decoc-tion can effectively alleviate intestinal inflammation and promote gastrointestinal functional recovery in POI rats via regula-ting macrophage polarization and the p38 MAPK signaling pathway.
