目的 探究膈下逐瘀汤加减方通过调控ROS/P38MAPK通路改善气滞血瘀型输卵管炎性不孕(SOI)的作用机制.方法 将80只SD雌性大鼠随机分为空白组、模型组、膈下逐瘀汤加减方组和阳性对照组.膈下逐瘀汤加减方组予中药方剂(2.16 g·kg-1·d-1),阳性对照组予左氧氟沙星片(0.045 g·kg-1·d-1),连续灌胃28天.观察各组大鼠输卵管组织情况;评价大鼠气滞血瘀证候积分;HE染色观察大鼠输卵管病理学改变;Western Blot检测大鼠输卵管BAX、Bcl-2、Caspase-3、Cleaved-Caspase-3、P38MAPK及P-P38MAPK蛋白表达情况;TUNEL染色检测各组大鼠输卵管组织凋亡情况;Elisa检测大鼠血清IL-1β、IL-10 及大鼠输卵管组织 ROS、SOD、MDA 表 达;RT-qPCR 检测大鼠输卵管组织中 BAX、Bcl-2、Caspase-3 及P38MAPK mRNA表达水平;观察各组大鼠受孕率.结果 模型组大鼠输卵管管腔大小不均,管壁增厚,气滞血瘀中医证候积分显著升高;膈下逐瘀汤加减方组大鼠输卵管组织及病理形态明显改善,中医证候积分显著下降,受孕率提高.模型组大鼠输卵管组织中凋亡、氧化应激水平及炎性因子表达显著上升(P<0.01),膈下逐瘀汤加减方治疗后明显下降(P<0.01,P<0.05).结论 膈下逐瘀汤加减方能够改善气滞血瘀型SOI大鼠输卵管组织形态,减轻炎症反应及氧化损伤,并通过调节ROS/P38MAPK信号的传导,抑制输卵管细胞凋亡,延缓疾病进展,恢复输卵管功能,提高受孕率.

Objective To investigate the therapeutic mechanism of Jiajian Gexia Zhuyu Decoction(膈下逐瘀汤加减方,JGZD)in ameliorating salpingitis inflammatory infertility(SOI)with qi stagnation and blood stasis syndrome via the reactive oxygen species(ROS)/p38 mitogen-activated protein Kinase(P38MAPK)pathway.Methods Eighty female Sprague-Dawley rats were randomly assigned to four groups:blank control,model(SOI),JGZD,and positive control(levofloxacin).The JGZD group was administered the decoction at 2.16 g·kg-1·d-1,and the positive control group received levofloxacin at 0.045 g·kg-1·d-1 via intragastric gavage for 28 con-secutive days.Upon completion,fallopian tube morphology was examined.The TCM syndrome score for qi stagnation and blood stasis was evaluated.Pathological changes were observed via hematoxylin-eosin(HE)staining.Protein expression of Bax,Bcl-2,Caspase-3,Cleaved Caspase-3,p38 MAPK,and p-p38 MAPK in fallopian tubes was detected by Western blot(WB).Apoptosis was assessed by terminal deoxynucleotidyl transferase dUTP nick end labeling(TUNEL)staining.Serum levels of IL-1β,IL-10,and ROS,as well as fallopian tube levels of superoxide dismutase(SOD)and malondialdehyde(MDA),were measured by enzyme-linked immunosorbent assay(ELISA).mRNA expression of Bax,Bcl-2,Caspase-3,and p38 MAPK was quantified by reverse transcription quantitative poly-merase chain reaction(RT-qPCR).Pregnancy rates were recorded.Results The model group exhibited uneven luminal dilation and wall thickening in fallopian tubes,along with a significantly increased TCM syndrome score.The JGZD group showed markedly improved tubal pathology,a significantly reduced syndrome score,and an increased pregnancy rate.Compared with the blank control,the model group demonstrated significantly elevated levels of apoptosis,oxidative stress markers(ROS,MDA;decreased SOD),and inflammatory cytokines(IL-1β;decreased IL-10)in fallopian tubes(P<0.01).These aberrant increases were significantly reversed by JGZD treatment(P<0.01,P<0.05).Conclusion JGZD ameliorated tubal morphology and function in rats with SOI and qi stagnation and blood stasis syndrome.Its mechanisms involve alleviating inflammation and oxidative damage,and inhibiting fallopian tube cell apoptosis via modulation of the ROS/P38MAPK signaling pathway,thereby delaying disease progression and improving fertility.
