Toll样受体基因多态性及其相乘交互效应与儿童反复肺炎的关联研究OA
Association of Toll-Like Receptor Gene Polymorphisms and Their Multiplicative Interaction Effects with Recurrent Pneumonia in Children
目的 分析Toll样受体(TLR)基因多态性及其相乘交互效应与儿童反复肺炎(RP)的关系.方法 本研究采用前瞻性对照研究设计,选取2022年1月—2024年6月在苏州市立医院儿科进行诊治的社区获得性肺炎(CAP)患儿99例为研究对象.收集患儿一般资料,检测TLR基因多态性(TLR2基因rs1898830、rs5743708位点,TLR3基因rs3775291、rs5752024位点,TLR4基因rs4986790、rs5030728位点基因型、等位基因频率).对患儿进行随访,终点事件为RP,共随访12个月.根据RP发生情况将患儿分为复发组和对照组.采用多因素Logistic回归分析探讨各TLR基因单核苷酸多态性(SNP)位点多态性及其相乘交互效应与儿童RP的关系.结果 99例患儿中,发生RP 49例(49.5%).两组TLR2基因rs1898830、rs5743708位点及TLR3基因rs5752024位点、TLR4基因rs5030728位点基因型分布及等位基因频率比较,差异有统计学意义(P<0.05).多因素Logistic回归分析结果显示:未纳入交互项时,携带TLR2基因rs1898830位点G等位基因、TLR2基因rs5743708位点C等位基因、TLR3基因rs3775291位点A等位基因、TLR4基因rs4986790位点A等位基因、TLR3基因rs5752024位点G等位基因、TLR4基因rs5030728位点C等位基因与儿童RP风险独立相关(P<0.05);纳入交互项后,TLR2基因rs1898830位点与TLR2基因rs5743708位点、TLR3基因rs3775291位点与TLR4基因rs4986790位点、TLR3基因rs5752024位点与TLR4基因rs5030728位点间存在相乘交互效应(P交互<0.05).结论 携带TLR2基因rs1898830位点G等位基因、TLR2基因rs5743708位点C等位基因、TLR3基因rs3775291位点A等位基因、TLR4基因rs4986790位点A等位基因、TLR3基因rs5752024位点G等位基因、TLR4基因rs5030728位点C等位基因与儿童RP风险独立相关;且TLR2基因rs1898830位点与TLR2基因rs5743708位点、TLR3基因rs3775291位点与TLR4基因rs4986790位点、TLR3基因rs5752024位点与TLR4基因rs5030728位点间存在相乘交互效应.
Objective To investigate the association of Toll-like receptor(TLR)gene polymorphisms and their multiplicative interaction effects with recurrent pneumonia(RP)in children.Methods In this study,a prospective controlled study design was used to select 99 children with community-acquired pneumonia(CAP)who were diagnosed and treated in the Department of Pediatrics,Suzhou Municipal Hospital from January 2022 to June 2024 as the study subjects.The general data of the children were collected,and the TLR gene polymorphism(genotype and allele frequency of TLR2 gene rs1898830 and rs5743708 locus,TLR3 gene rs3775291 and rs5752024 locus,TLR4 gene rs4986790 and rs5030728 locus)was detected.The children were followed up for a total of 12 months,and the endpoint event was RP.According to the occurrence of RP,the children were divided into the recurrence group and the control group.Multivariate Logistic regression analysis was used to explore the association of TLR gene single nucleotide polymorphism(SNP)locus polymorphism and their multiplicative interaction effects with RP in children.Results Among the 99 children,49(49.5%)developed RP.There were significant differences in genotype distribution and allele frequency of TLR2 gene rs1898830 and rs5743708 locus,TLR3 gene rs5752024 locus and TLR4 gene rs5030728 locus between the two groups(P<0.05).Multivariate Logistic regression analysis showed that when the interaction term was not included,the G allele of TLR2 gene rs1898830 locus,the C allele of TLR2 gene rs5743708 locus,the A allele of TLR3 gene rs3775291 locus,the A allele of TLR4 gene rs4986790 locus,the G allele of TLR3 gene rs5752024 locus and the C allele of TLR4 gene rs5030728 locus were independently associated with the risk of RP in children(P<0.05);after the interaction term was included,there was a multiplicative interaction between the TLR2 gene rs1898830 locus and the TLR2 gene rs5743708 locus,between the TLR3 gene rs3775291 locus and the TLR4 gene rs4986790 locus,between the TLR3 gene rs5752024 locus and the TLR4 gene rs5030728 locus(Pinteraction<0.05).Conclusion Carrying TLR2 gene rs1898830 locus G allele,TLR2 gene rs5743708 locus C allele,TLR3 gene rs3775291 locus A allele,TLR4 gene rs4986790 locus A allele,TLR3 gene rs5752024 locus G allele,TLR4 gene rs5030728 locus C allele are independently associated with RP risk in children.There are multiplicative interaction effects between TLR2 gene rs1898830 locus and TLR2 gene rs5743708 locus,TLR3 gene rs3775291 locus and TLR4 gene rs4986790 locus,TLR3 gene rs5752024 locus and TLR4 gene rs5030728 locus.
