目的 分析心肌肌钙蛋白I(cTnI)变异性与急性心肌梗死(AMI)患者经皮冠状动脉介入治疗(PCI)后发生主要不良心脑血管事件(MACCE)的关系.方法 本研究采用前瞻性队列研究设计,连续选取2019-06-30至2023-06-30新疆医科大学第一附属医院心脏中心收治的行急诊PCI的AMI患者816例为研究对象.收集患者的临床资料,记录患者入院时cTnI及其变异系数.出院后对患者进行电话随访,每年随访1次,随访至2025-12-01,记录MACCE发生情况.采用Kaplan-Meier法绘制生存曲线,生存曲线间的比较采用Log-rank检验;采用单因素、多因素Cox回归分析探讨AMI患者PCI后发生MACCE的影响因素;采用限制性立方样条(RCS)曲线分析cTnI变异系数与AMI患者PCI后MACCE发生风险间的剂量-反应关系.结果 根据cTnI变异系数三分位数将患者均分为低变异性组(<0.796)、中变异性组(0.796~1.336)和高变异性组(>1.336),每组272例.三组高血压发生率和入院时血红蛋白、白细胞计数、TG、白蛋白、AST、cTnI及cTnI变异系数比较,差异有统计学意义(P<0.05).816例患者中,发生MACCE 252例(30.88%).低变异性组、中变异性组累积MACCE发生率高于高变异性组(P<0.05).单因素Cox回归分析结果显示,高血压、入院时血小板计数、入院时肌酐、cTnI变异性可能是AMI患者PCI后发生MACCE的影响因素(P<0.05);多因素Cox回归分析结果显示,入院时血小板计数、cTnI变异性是AMI患者PCI后发生MACCE的独立影响因素(P<0.05).RCS曲线分析结果显示:cTnI变异系数与AMI患者PCI后MACCE发生风险呈线性负相关(P整体=0.014,P非线性=0.148);当cTnI变异系数>1.17时,随着cTnI变异系数增大,AMI患者PCI后MACCE发生风险呈持续、单调的下降趋势.结论 cTnI低变异性、中变异性是AMI患者PCI后发生MACCE的独立危险因素,且cTnI变异系数与AMI患者PCI后MACCE发生风险呈线性负相关.

Objective To investigate the relationship between cardiac troponin I(cTnI)variability and major adverse cardiovascular and cerebrovascular events(MACCE)in patients with acute myocardial infarction(AMI)after percutaneous coronary intervention(PCI).Methods In this study,a prospective cohort study design was used to continuously select 816 patients with AMI who underwent emergency PCI in the Cardiovascular Center of the First Affiliated Hospital of Xinjiang Medical University from June 30,2019 to June 30,2023 as the study subjects.The clinical data of the patients were collected,and the cTnI and its coefficient of variation at admission were recorded.After discharge,the patients were followed up by telephone once a year until 2025-12-01,and the occurrence of MACCE was recorded.Kaplan-Meier method was used to draw the survival curve,and Log-rank test was used to compare the survival curves.Univariate and multivariate Cox regression analysis was used to explore the influencing factors of MACCE in AMI patients after PCI.The dose-response relationship between the coefficient of variation of cTnI and the risk of MACCE after PCI in AMI patients was analyzed by restricted cubic spline(RCS)curve.Results According to the tertile of cTnI coefficient of variation,the patients were divided into the low variability group(<0.796),the medium variability group(0.796-1.336)and the high variability group(>1.336),with 272 cases in each group.There were significant differences in the incidence of hypertension,hemoglobin,white blood cell count,TG,albumin,AST and cTnI at admission,and cTnI coefficient of variation among the three groups(P<0.05).Among the 816 patients,MACCE occurred in 252 cases(30.88%).The cumulative incidence of MACCE in the low variability group and the medium variability group was higher than that in the high variability group(P<0.05).Univariate Cox regression analysis showed that hypertension,platelet count at admission,creatinine at admission,and cTnI variability may be the influencing factors of MACCE in AMI patients after PCI(P<0.05).Multivariate Cox regression analysis showed that platelet count at admission and cTnI variability were independent influencing factors of MACCE in AMI patients after PCI(P<0.05).The results of RCS curve analysis showed that the coefficient of variation of cTnI was negatively correlated with the risk of MACCE in AMI patients after PCI(Poverall=0.014,Pnon-linear=0.148).When the coefficient of variation of cTnI>1.17,with the increase of the coefficient of variation of cTnI,the risk of MACCE in AMI patients after PCI showed a continuous and monotonous downward trend.Conclusion Low variability and moderate variability of cTnI are independent risk factors for MACCE in AMI patients after PCI,and the coefficient of variation of cTnI is linearly negatively correlated with the risk of MACCE in AMI patients after PCI.
