幽门螺杆菌感染对成人同型半胱氨酸水平及脉搏波传导速度的影响OA
Effect of helicobacter pylori infection on adult homocysteine level and pulse wave velocity
目的:探讨幽门螺杆菌感染与血清同型半胱氨酸(Hcy)水平及肱踝脉搏波传播速度(baPWV)之间的关联,评估幽门螺杆菌感染是否为高 Hcy血症和动脉僵硬的独立风险因素.方法:纳入 2022年 1月—2023年 12月在我院接受体检的 6 207名成年人.根据 13C尿素呼气试验结果将受试者分为幽门螺杆菌阳性组与幽门螺杆菌阴性组.采用线性回归分析评估幽门螺杆菌感染与Hcy及baPWV的线性关联.进一步采用多变量逻辑回归分别构建高Hcy(≥15 μmol/L)和高baPWV(≥1 800 cm/s)模型,并进行敏感性分析.结果:幽门螺杆菌感染与Hcy或baPWV均无明显线性关联.高Hcy增加幽门螺杆菌阳性个体发生风险(P<0.001),且发生高baPWV的风险明显升高(P<0.001),敏感性分析结果一致.结论:幽门螺杆菌感染与Hcy或baPWV无明显线性关联,但幽门螺杆菌感染明显增加高Hcy血症及动脉僵硬风险,幽门螺杆菌感染可能通过炎症反应、营养吸收障碍或代谢途径影响血管弹性.
Objective:To investigate the association between helicobacter pylori infection and serum homocysteine(Hcy)levels as well as brachial-ankle pulse wave velocity(baPWV),and to assess whether helicobacter pylori infection is an independent risk factor for hyperhomocysteinemia and arterial stiffness.Methods:A total of 6 207 adults who underwent physical examinations at our hospital from January 2022 to December 2023 were included.The subjects were divided into the helicobacter pylori positive group and the helicobacter pylori negative group based on the results of the 13C urea breath test.Linear regression analysis was used to evaluate the linear association between helicobacter pylori infection and Hcy as well as baPWV.Further,multivariate logistic regression was employed to construct models for high Hcy(≥15 μmol/L)and high baPWV(≥1 800 cm/s),and sensitivity analysis was conducted.Results:There was no significant linear correlation between helicobacter pylori infection and Hcy or baPWV.High Hcy increased the risk of helicobacter pylori positive individuals(P<0.001),and the risk of having high baPWV was significantly increased(P<0.001).The results of the sensitivity analysis were consistent.Conclusions:There is no significant linear correlation between helicobacter pylori infection and Hcy or baPWV.However,helicobacter pylori infection significantly increases the risk of high Hcy levels and arterial stiffness.Helicobacter pylori infection may affect vascular elasticity through inflammatory responses,nutrient absorption disorders,or metabolic pathways.
