目的:探讨脑卒中患者睡眠相关运动障碍(SRMDs)的患病率,并分析其影响因素及对神经功能康复的作用.方法:选取2021年6月至2024年5月江苏省南通市海门区人民医院收治的急性脑卒中患者108例作为研究对象.按照入院后14 d内,是否合并SRMDs分为SRMDs组与非SRMDs组.结果:脑卒中患者SRMDs的患病率为41.67%;SRMDs组与非SRMDs组在年龄、脑卒中类型、病变部位、美国国立卫生研究院卒中量表(NIHSS)评分、焦虑抑郁、匹兹堡睡眠质量指数(PSQI)总分、呼吸事件指数、阻塞性呼吸暂停指数、低通气指数、夜间最低血氧饱和度、血氧饱和度<90%时间占比方面比较,差异均有统计学意义(均P<0.05);结果显示,年龄、脑卒中类型、病变部位、NIHSS评分、焦虑抑郁、夜间疼痛、PSQI总分、夜间最低血氧饱和度是脑卒中患者发生SRMDs的独立影响因素,差异有统计学意义(P<0.05);入院时、出院后3个月SRMDs组NIHSS评分均高于非SRMDs组,差异有统计学意义(P<0.05),入院时2组FMA评分、Barthel指数比较,差异无统计学意义(P>0.05),出院后3个月2组运动功能评分(FMA)评分、Barthel指数均升高,SRMDs组低于非SRMDs组,差异有统计学意义(P<0.05).结论:脑卒中后SRMDs患病率高,其发生与高龄、特定卒中部位、神经功能缺损严重度、不良情绪及睡眠异常相关,且合并SRMDs阻碍了卒中患者的神经功能恢复进程.

Objective:To investigate the prevalence of sleep-related Movement Disorders(SRMDs)in stroke patients,and analyze their influencing factors and their effects on neurological functional recovery.Methods:A total of 108 patients with acute stroke admitted to the neurology department of our hospital from June 2021 to May 2024 were included.Within 14 days after admission,patients were divided into SRMDs group and non SRMDs group based on the presence of SRMDs.Results:The prevalence of SRMDs was 41.67%.Significant differences were observed between the two groups in age,stroke type,lesion location,National Institutes of Health Stroke Scale(NIHSS)score,anxiety/depression,total Pittsburgh Sleep Quality Index(PSQI)score,respiratory event index,obstructive apnea index,hypopnea index,nocturnal minimum oxygen saturation,and percentage of time with SpO2<90%(all P<0.05).Multivariate analysis identified age,stroke type,lesion location,NIHSS score,anxiety/depression,nocturnal pain,total PSQI score,and nocturnal minimum oxygen saturation as independent influencing factors for SRMDs(P<0.05).At ad-mission and at 3 months post-discharge,the SRMDs group had significantly higher NIHSS scores than the non-SRMDs group(P<0.05).No significant differences were found between the two groups in Fugl-meyer Assessment(FMA)score or Barthel index at admission(P>0.05);however,at 3 months post-discharge,both measures improved in both groups,with significantly lower scores in the SRMDs group(P<0.05).Conclusion:Post-stroke SRMDs have a high prevalence,and their occurrence is associat-ed with older age,specific lesion locations,greater neurological deficits,negative emotional states,and sleep disturbances.Moreo-ver,comorbid SRMDs impede the recovery of neurological function.Early identification and management of SRMDs are recommen-ded for clinical practice.
