目的:分析非痴呆帕金森病(PD)患者的睡眠障碍与认知功能障碍相关性.方法:选取2023年6月至2025年12月天水市第一人民医院神经内科收治的非痴呆型帕金森病患者100例为研究对象,根据是否伴有轻度认知障碍(MCI)将其分为伴有MCI的帕金森病组(观察组,n=47)和不伴有MCI的帕金森病组(对照组,n=53).全部患者均完成了视频多导睡眠监测检查,同时运用Epworth嗜睡量表(ESS)对其日间嗜睡状况进行评估.结果:2组年龄、性别、受教育年限、药物治疗等情况,差异无统计学意义(P>0.05);2组病程、运动障碍协会统一帕金森病评定量表(MDS-UPDRS)评分比较,差异有统计学意义(P<0.05);2组总睡眠时间、觉醒时间、睡眠潜伏时间、快速眼动睡眠(REM)N1期占比及REM占比比较,差异有统计学意义(P<0.05);观察组睡眠障碍发生率高于对照组,且快速眼动期睡眠行为障碍发生率高于对照组,差异有统计学意义(P<0.05);观察组ESS评分高于对照组,差异有统计学意义(P<0.05).Logistic回归分析显示,病程、总睡眠时间、觉醒时间、NREM N1期占比、REM占比、快速眼动期睡眠行为障碍、睡眠障碍数目是PD患者发生MCI的独立影响因素(P<0.05).结论:PD并MCI患者无一例外均存在睡眠障碍,睡眠障碍可能在PD患者认知损害的调控中发挥关键作用.

Objective:To analyze the correlation between sleep disorders and cognitive dysfunction in non-demented Parkinson's disease(PD)patients.Methods:A total of 100 patients with non-demented Parkinson's disease admitted to the Department of Neurology at Tianshui First People's Hospital from June 2023 to December 2025 were selected as the research subjects.Based on whether they were accompanied by mild cognitive impairment(MCI),they were divided into two groups:an observation group(PD with MCI,n=47)and a control group(PD without MCI,n=53).All patients underwent video polysomnography(vPSG)and their daytime sleepiness was assessed using the Epworth Sleepiness Scale(ESS).Results:There were no statistically significant differ-ences in age,gender,years of education,and medication treatment between the two groups(P>0.05).However,the differences in disease duration and the Movement Disorder Society-Unified Parkinson's Disease Rating Scale(MDS-UPDRS)scores were statis-tically significant(P<0.05).Statistically significant differences were also observed between the two groups in terms of total sleep time,wake time,sleep latency,the proportion of non-rapid eye movement(NREM)N1 stage,and the proportion of rapid eye move-ment(REM)sleep(P<0.05).The incidence of sleep disorders was higher in the observation group than that in the control group,and the incidence of rapid eye movement sleep behavior disorder(RBD)was also higher than that in the control group,with statistically significant differences(P<0.05).In addition,the ESS scores in the observation group were higher than those in the control group,with a statistically significant difference(P<0.05).Binary logistic regression analysis revealed that disease dura-tion,total sleep time,wake time,the proportion of NREM N1 stage,the proportion of REM sleep,RBD,and the number of sleep disorders were independent influencing factors for the development of MCI in PD patients(P<0.05).Conclusion:All PD pa-tients with MCI invariably have sleep disorders,and sleep disorders may play a crucial role in the regulation of cognitive impair-ment in PD patients.
