Objective To explore the effects of controlling fetal head delivery speed in the second stage of la-bor based on perineal central tendon tension on delivery outcomes in primiparas undergoing vaginal delivery.Methods A total of 680 primiparas scheduled for vaginal delivery in the obstetrics department at a hospital in Shanghai from November 2024 to April 2025 were selected by convenience sampling.They were divided into the control group(n=340)and the observation group(n=340)according to different criteria for controlling fetal head delivery speed.In the control group,the fetal head delivery speed was controlled by limiting the fetal head exposed at the vaginal introitus to<1 cm during pushing;in the observation group,the speed was con-trolled based on the tension of the perineal central tendon.The two groups were compared in terms of perineal laceration rate,neonatal asphyxia rate,intrapartum blood loss,and perineal pain scores.Results The observa-tion group showed lower perineal laceration and 1-min neonatal asphyxia rates(P<0.05),and lower intrapartum and 2-h postpartum blood loss and pain scores than the control group(P<0.05).Conclusion Controlling fetal head delivery speed based on the tension of the perineal central tendon in primiparas undergoing vaginal deliv-ery can reduce the incidence of adverse maternal-neonatal outcomes and alleviate maternal pain,demonstrat-ing high practical value.
