从"气"探析心系疾病与线粒体功能障碍关系OA

Exploring relationship between cardiovascular diseases and mitochondrial dysfunction from perspective of Qi

心系疾病的发生发展与心肌能量代谢、氧化还原稳态、钙信号调控及细胞死亡过程密切相关.线粒体作为物质代谢和能量转换的核心细胞器,其功能障碍贯穿动脉粥样硬化、冠心病、心律失常、心力衰竭等病变过程.线粒体DNA异常、氧化磷酸化受损、活性氧过量生成、钙稳态失衡、线粒体自噬及分裂融合失调,可造成心肌细胞结构和功能损伤,并与疾病进展形成相互促进的病理循环.气为生命活动之本,具有推动、温煦、防御、固摄和气化等功能,其生成依赖先天之精、水谷精微与自然清气,运行则体现为升降出入及脏腑经络的协同作用.基于中西医理论互参,线粒体与"气"在源、用、机、治层面具有一定相通性.线粒体作为细胞稳态调控的重要枢纽,从微观层面阐释气的化生、运行及充养心脉之功;其功能失衡所致心肌供能不足、内环境紊乱和损伤累积,可与心气不足、气机失调、气虚血瘀及脏腑气化失司等病机相互印证.从气论治心系疾病应结合疾病类型、证候特点及线粒体损伤环节,分别从调补宗气、平衡阴阳二气、调和营卫、整体调节脏腑之气等方面确立治法,改善线粒体生物合成、能量代谢、氧化应激、膜电位稳定性及质量控制.该理论框架有助于阐释中医辨证论治心系疾病的微观基础,为中西医协同干预及后续机制研究提供思路.

Occurrence and progression of cardiovascular diseases were closely associated with myocardial ener-gy metabolism,redox homeostasis,calcium signaling regulation,and cell death.As core organelle involved in metabo-lism and energy conversion,mitochondrial dysfunction was involved throughout pathological processes of atheroscle-rosis,coronary heart disease,arrhythmia,and heart failure.Mitochondrial DNA abnormalities,impaired oxidative phosphorylation,excessive reactive oxygen species production,calcium homeostasis imbalance,and dysregulation of mitophagy and mitochondrial fission-fusion dynamics could jointly lead to structural and functional injury of cardio-myocytes and form a mutually reinforcing pathological cycle with disease progression.Qi was regarded as the basis of life activities and had functions of promotion,warming,defense,consolidation,and transformation.Its generation de-pends on innate essence,essence of water and grain,and natural clear Qi,while its movement was manifested as as-cending,descending,entering,and exiting,as well as the coordinated action of the Zangfu organs and meridians.Based on integrative understanding of Chinese and Western medicine,mitochondria and Qi share certain similarities in ori-gin,function,pathogenesis,and treatment.As an important hub of cellular homeostasis regulation,mitochondria may provide a microscopic explanation for the generation and movement of Qi and its role in nourishing the heart vessels.Mitochondrial dysfunction-induced myocardial energy deficiency,homeostatic disturbance,and cumulative injury can correspond to such pathological changes as heart Qi deficiency,Qi movement disorder,Qi deficiency and blood stasis,and dysfunction of Zangfu Qi transformation.Accordingly,Qi-based treatment of cardiovascular diseases should be established according to disease type,syndrome characteristics,and specific links of mitochondrial injury.Therapeutic strategies may include supplementing and regulating Zong Qi,balancing Yin and Yang Qi,harmonizing nutrient and defensive Qi,and integrally regulating Zangfu Qi.To improve mitochondrial biogenesis,energy metabolism,oxidative stress,mitochondrial membrane potential stability,and quality control.This theoretical framework may help elucidate the microscopic basis of syndrome differentiation and treatment of cardiovascular diseases in traditional Chinese med-icine and provide ideas for integrated Chinese and western medical intervention and subsequent mechanistic studies.
