基于PI3K/Akt/NF-κB通路探讨羌活膏摩法对膝骨关节炎大鼠作用机制OA

目的:探讨羌活膏摩法改善膝骨关节炎(KOA)作用机制.方法:通过膝关节腔注射木瓜蛋白酶方式作用于SD大鼠,建立KOA动物模型.于造模完成与治疗结束后行Lequesne MG评分评估膝关节功能.通过膝关节滑膜组织HE染色观察软骨及滑膜组织形态变化;ELISA检测血清IL-1β、TNF-α、IL-10表达水平;Western blot检测膝关节滑膜组织PI3K-Akt通路相关蛋白(p-PI3K/PI3K、p-Akt/Akt、p-NF-KB/NF-KB)表达.实时荧光PCR法检测滑膜IL-4、IL-10、COMP基因表达水平.结果:行为学显示,与模型组比较,治疗组大鼠Lequesne MG评分降低(P<0.001),与空白组比较,差异无统计学意义(P>0.05).HE染色提示经羌活膏摩法干预后大鼠膝关节软骨损伤程度减轻,滑膜炎症浸润减少.ELISA表明,与模型组相比,治疗组血清中促炎因子IL-1β、TNF-α含量降低,抗炎因子IL-10含量升高(P<0.001).Western blot和RT-qPCR结果显示,治疗组大鼠膝关节滑膜组织中p-PI3K、p-Akt及NF-κB蛋白表达下调(P<0.05),滑膜组织中IL-4、IL-10 mRNA较模型组升高,COMP mRNA表达降低(P<0.05).结论:羌活膏摩法可有效降低KOA大鼠模型炎症,改善关节功能,其机制可能与抑制PI3K/Akt/NF-κB信号通路活化相关.

Objective:To explore the mechanism of Qianghuo ointment Tuina in improving knee osteoarthritis(KOA).Methods:The KOA model was established in SD rats by intra-articular injection of papain.Lequesne MG score was used to evaluate knee joint function after model establishment and treatment completion.HE staining of knee synovial tissue was performed to observe morphological changes of cartilage and synovium.The serum levels of IL-1β,TNF-α and IL-10 were detected by ELISA.Expression of PI3K-Akt pathway-related proteins(p-PI3K/PI3K,p-Akt/Akt,p-NF-KB/NF-KB)in knee synovial tissue was measured by Western blot.mRNA expression levels of IL-4,IL-10 and COMP in synovium were determined by real-time fluorescent quantitative PCR.Results:Behavioral results showed that compared with the model group,Lequesne MG score was significantly decreased in the treatment group(P<0.001),with no statistically significant difference compared with the blank group(P>0.05).HE staining re-vealed alleviated cartilage damage and reduced synovial inflammatory infiltration in rats after intervention with Qian-ghuo ointment Tuina.ELISA results demonstrated that compared with the model group,the serum contents of pro-inflammatory factors IL-1β and TNF-α were notably decreased,while the content of anti-inflammatory factor IL-10 was remarkably increased in the treatment group(P<0.001).Western blot and RT-qPCR results indicated that pro-tein expression levels of p-PI3K,p-Akt and NF-κB in knee synovial tissue were significantly down-regulated(P<0.05),mRNA levels of IL-4 and IL-10 were elevated,and mRNA expression of COMP was markedly reduced in the treatment group relative to the model group(P<0.05).Conclusion:Qianghuo ointment Tuina can effectively attenu-ate inflammation and improve joint function in KOA rat models.Its mechanism may be related to the inhibition of PI3K/Akt/NF-κB signaling pathway activation.
