目的 探讨非肥胖人群的血浆致动脉粥样硬化指数(AIP)与糖尿病前期(PDM)的关系,及AIP对PDM的预测价值.方法 纳入2021年1月至2024年12月在我院健康管理中心体检的45260例非肥胖者,参照WHO糖尿病标准分为PDM组和糖耐量正常组(NG组).采用二元Logistic回归模型分析PDM的影响因素,受试者工作特征曲线(ROC曲线)评估AIP对PDM的预测作用.结果 ①PDM患病率为8.24%,PDM组AIP高于NG组(P<0.05).②随着AIP水平升高,PDM患病率也随之升高,且在高AIP状态下男性患PDM的风险更突出(P<0.05).③多因素Logistic回归显示,AIP水平与PDM风险呈显著正相关.在调整全部的危险因素后,AIP四分位最高组(Q4)较最低组(Q1)PDM风险增加82.4%(OR=1.842,95%CI 1.724~1.978,P<0.001);男性、年龄、SBP、BMI、ALT、AST、GGT、ALB、BUN、TG、FBG、HbA1c及AIP为PDM的独立危险因素,而ApoA为独立保护因素.④ROC曲线显示,AIP预测PDM的曲线下面积为0.729(95%CI 0.720~0.738,P<0.05),最佳诊断阈值为-0.064.结论 AIP是非肥胖体检人群PDM的独立危险因素,对PDM具有一定预测价值,ApoA为该人群PDM的独立保护因素.

Objective To investigate the relationship between the atherogenic index of plasma(AIP)and prediabetes mellitus(PDM)in non-obese individuals,and to evaluate the predictive value of AIP for PDM.Methods A total of 45,260 non-obese subjects who underwent physical examinations at the Health Management Center of our hospital from January 2021 to December 2024 were enrolled.Subjects were divided into the PDM group and normal glucose tolerance(NG)group according to the WHO diabetes diagnostic criteria.Binary logistic regression model was used to analyze the influencing factors of PDM,and re-ceiver operating characteristic(ROC)curve was employed to assess the predictive value of AIP for PDM.Results ①The preva-lence of PDM was 8.24%,and the AIP level in the PDM group was significantly higher than that in the NG group(P<0.05).②With the increase of AIP level,the prevalence of PDM gradually increased,and the risk of PDM in males was more prominent in the high AIP state(P<0.05).③Multivariate logistic regression analysis showed that AIP level was significantly positively correlated with the risk of PDM.After adjusting for all confounding factors,compared with the lowest quartile(Q1),the highest quartile(Q4)of AIP was associated with an 82.4%increased risk of PDM(OR=1.842,95%CI 1.724~1.978,P<0.001).Male gender,age,systolic blood pressure(SBP),body mass index(BMI),alanine aminotransferase(ALT),aspartate aminotransferase(AST),gam-ma-glutamyl transferase(GGT),albumin(ALB),blood urea nitrogen(BUN),triglycerides(TG),fasting blood glucose(FBG),glycated hemoglobin(HbA1c),and AIP were independent risk factors for PDM,while apolipoprotein A(ApoA)was an independ-ent protective factor.④ROC curve analysis showed that the area under the curve(AUC)of AIP for predicting PDM was 0.729(95%CI 0.720~0.738,P<0.05),with the optimal diagnostic cutoff value of-0.064.Conclusion AIP is an independent risk factor for PDM in non-obese physical examination populations and has certain predictive value for PDM.ApoA is an independent protective factor for PDM in this population.
