目的 探讨2型糖尿病(T2DM)合并代谢相关脂肪性肝病(MAFLD)患者血清miR-122、烟酰胺腺嘌呤二核苷酸磷酸氧化酶2(NOX2)、β-抑制蛋白2(β-arrestin2)水平与胰岛素抵抗(IR)和肝纤维化的关系.方法 纳入2022年2月至2025年4月我院收治的T2DM患者146例,其中合并MAFLD患者91例(MAFLD组),无MAFLD患者55例(非MAFLD组).RT-qPCR法检测血清miR-122表达水平;ELISA法检测血清NOX2、β-arrestin 2水平;Pearson法分析T2DM合并MAFLD患者血清miR-122、NOX2、β-arrestin 2与IR指标的相关性;Logistic回归分析T2DM合并MAFLD患者发生肝纤维化的影响因素;ROC曲线评估血清miR-122、NOX2、β-arrestin 2对T2DM合并MAFLD患者发生肝纤维化的诊断效能.结果 MAFLD组患者体质指数、甘油三酯(TG)、天冬氨酸氨基转移酶(AST)、谷氨酰基转移酶(GGT)、丙氨酸氨基转移酶(ALT)、糖化血红蛋白(HbA1c)、空腹血糖(FBG)、空腹胰岛素(FINS)、稳态模型IR指数(HOMA-IR)、肝脏硬度(LSM)、控制衰减指数(CAP)及血清miR-122、NOX2、β-arrestin 2水平明显高于非MAFLD组,高密度脂蛋白胆固醇(HDL-C)、25-羟维生素D(25-(OH)D)水平明显低于非MAFLD组(P<0.05).轻度和中重度纤维化组患者HOMA-IR、LSM、CAP及血清miR-122、NOX2、β-arrestin 2水平明显高于无纤维化组,且轻度和中重度纤维化组差异有统计学意义(P<0.05).T2DM 合并 MAFLD 患者血清 miR-122、NOX2、β-arrestin 2 水平与 FINS、HOMA-IR 呈正相关(P<0.05).HO-MA-IR、LSM、CAP及血清miR-122、NOX2、β-arrestin 2水平升高是影响肝纤维化发生的危险因素(P<0.05).血清miR-122、NOX2、β-arrestin 2联合诊断T2DM合并MAFLD患者发生肝纤维化的AUC显著高于各自独自检测(Z三者联合-miR-122=2.075、Z三者联合-NOX2=2.383、Z三者联合-β-arrestin 2=2.566,P<0.05).结论 T2DM 合并 MAFLD 患者血清 miR-122、NOX2、β-ar-restin 2水平升高,且三者均与IR和肝纤维化显著相关,其联合检测诊断肝纤维化的价值较高.

Objective To explore relationships between serum miR-122,nicotinamide adenine dinucleotide phosphate ox-idase 2(NOX2),β-arrestin 2 levels and insulin resistance(IR)and liver fibrosis in patients with type 2 diabetes mellitus(T2DM)complicated with metabolic dysfunction-associated fatty liver disease(MAFLD).Methods From February 2022 to April 2025,total 146 T2DM patients were included,including 91 patients with MAFLD(MAFLD group)and 55 patients without MAFLD(non-MAFLD group).The RT-qPCR method was used to detect serum miR-122.ELISA method was performed to detect serum NOX2 and β-arrestin 2.Pearson method was performed to explore the correlation between serum miR-122,NOX2,β-arrestin 2,and IR in patients with T2DM complicated with MAFLD.Logistic regression was used to explore the influencing factors of liver fibrosis in patients with T2DM complicated with MAFLD.In addition,ROC curve was used to evaluate the diagnostic efficacy of serum miR-122,NOX2,and β-arrestin 2 for liver fibrosis in patients with T2DM complicated with MAFLD.Results Patients in the MAFLD group exhibited significantly higher body mass index(BMI),triglycerides(TG),aspartate aminotransferase(AST),gam-ma-glutamyl transferase(GGT),alanine aminotransferase(ALT),glycated hemoglobin(HbA1c),fasting blood glucose(FBG),fasting insulin(FINS),homeostasis model assessment-insulin resistance(HOMA-IR),liver stiffness measurement(LSM),control attenuation parameter(CAP),serum miR-122,NOX2 and β-arrestin 2 levels than those in the non-MAFLD group,while high-den-sity lipoprotein cholesterol(HDL-C)and 25-hydroxyvitamin D(25-(OH)D)levels were lower(P<0.05).The mild group and moderate to severe fibrosis group had prominently higher HOMA-IR,LSM,CAP,serum miR-122,NOX2,and β-arrestin 2 than the non-fibrosis group,and the differences between the mild group and moderate to severe fibrosis group were prominent(P<0.05).Serum miR-122,NOX2,and β-arrestin 2 in patients with T2DM complicated with MAFLD were positively correlated with FINS and HOMA-IR(P<0.05).Elevated HOMA-IR,LSM,CAP,serum miR-122,NOX2,and β-arrestin 2 were risk factors for the occur-rence of liver fibrosis(P<0.05).The combination of serum miR-122,NOX2,and β-arrestin 2 had a clearly higher AUC in diag-nosing liver fibrosis in patients with T2DM complicated with MAFLD than their individual detection(Zcombination-miR-122=2.075,Zcombination-NOX2=2.383,Zcombination-β-arrestin2=2.566,P<0.05).Conclusion Serum miR-122,NOX2,and β-arrestin 2 are elevated in patients with T2DM complicated with MAFLD,and all three are clearly correlated with IR and liver fibrosis.In addition,their combined detection has high diagnostic value for liver fibrosis.
