透视辅助定位"理想"股骨止点重建术治疗前交叉韧带损伤的疗效分析OA
Efficacy Analysis of the Fluoroscopically Assisted"I.D.E.A.L."Femoral Insertion Reconstruction in the Treat-ment of Anterior Cruciate Ligament Injury
目的 探究透视辅助定位"理想"股骨止点重建术治疗前交叉韧带损伤的疗效.方法 回顾性分析2017年1月至2021年1月国家体育总局运动医学研究所体育医院收治的67例前交叉韧带损伤患者的临床资料,根据不同的手术重建方式分为观察组(n=32)和对照组(n=35),观察组行透视辅助下的"理想"股骨止点重建术,对照组患者行常规关节镜下前交叉韧带重建术.比较两组患者术后随访时的膝关节查体结果、膝关节功能评分、膝关节运动功能表现情况并观察术后并发症.结果 观察组随访时间为12~30个月,平均(21.44±4.91)个月,对照组随访时间为14~32个月,平均(22.26±5.68)个月.观察组术后Lysholm评分、IKDC评分、Marx评分均高于对照组(P<0.05);观察组术后单腿跳、三连跳、交叉三连跳测试患侧与健侧比值均优于对照组(P<0.05).两组患者术后膝关节查体结果(Lachman试验、轴移试验)、ACL-RSI评分、Y-balance测试患侧与健侧比值及术后并发症发生率比较,差异均无统计学意义(P>0.05).结论 透视辅助定位"理想"股骨止点重建术治疗前交叉韧带损伤,可以提高膝关节功能评分和运动功能表现,且安全可靠.
Objective To explore the efficacy of fluoroscopically assisted"I.D.E.A.L."femoral insertion reconstruction in the treatment of anterior cruciate ligament injury.Methods The clinical data of 67 patients with ACL injury admitted to the Sports Hospital,Institute of Sports Medicine,General Administration of Sport of China from January 2017 to January 2021 were ret-rospectively analyzed.According to different surgical reconstruction methods,they were divided into observation group(n=32)and control group(n=35).The observation group,underwent"1.D.E.A.L."femoral insertion reconstruction assisted by fluorosco-py.The control group underwent routine arthroscopic ACL reconstruction.The results of knee joint physical examination,knee joint function score and knee joint motor function were compared between the two groups at the postoperative follow-up time,and postop-erative complications were observed.Results The observation group was followed up for 12 to 30 months,the average(21.44±4.91)months,the control group was followed up for 14 to 32 months,the average(22.26±5.68).The postoperative Lysholm score,IKDC score,and Marx score in the observation group were significantly higher than those in the control group(all P<0.05).The ratios of the affected side to the healthy side in the single-leg hop test,triple hop test,and crossover triple hop test in the observation group were significantly better than those in the control group(all P<0.05).There were no statistically significant differences between the two groups in postoperative knee physical examination results(Lachman test,pivot shift test),ACL-RSI score,Y-balance test ratio(affected/healthy side),or the incidence of postoperative complications(all P>0.05).Conclusion Fluoroscopy-assisted"I.D.E.A.L."femoral insertion reconstruction for anterior cruciate ligament injury can improve knee func-tional score and motor function performance,and is safe and reliable.
