精细化护理对老年髋部骨折患者康复效果影响的Meta分析OA
Impact of refined nursing on rehabilitation outcomes in elderly patients with hip fracture:a Meta-analysis
目的 评估精细化护理对老年髋部骨折患者术后康复效果的影响.方法 系统检索 PubMed、Embase、Cochrane Library、Web of Science、CNKI、万方、维普等数据库,收集关于精细化护理在老年髋部骨折患者中应用的随机对照试验和回顾性队列研究,检索时限为建库至2026年1月31日.使用 Cochrane 偏倚风险工具和纽卡斯尔-渥太华量表分别评估随机对照试验和队列研究的质量.使用R4.2.3软件进行Meta分析.结果 共纳入25 项研究,共2157名老年髋部骨折患者.Meta分析结果显示:精细化护理组临床有效率(OR=5.52,95％CI:3.14～9.72,P<0.0001)、护理满意度(OR=3.36,95％CI:2.32～4.84,P<0.0001)显著高于常规护理组;并发症发生率(OR=0.21,95％CI:0.13～0.32,P<0.0001)、视觉模拟评分法评分(MD=-2.78,95％CI:-3.46～-2.11,P<0.0001)显著低于常规护理组.结论 精细化护理可显著改善老年髋部骨折患者的术后康复效果,在提高临床有效率、护理满意度,降低并发症发生率、缓解术后疼痛等方面均优于常规护理,具有较高的临床推广应用价值.
Objective To evaluate the effectiveness of refined nursing on postoperative rehabilitation outcomes in elderly patients with hip fracture.Methods A systematic search was conducted in databases including PubMed,Embase,the Cochrane Library,Web of Science,CNKI,Wanfang,and VIP to collect randomized controlled trials or retrospective cohort studies on the application of refined nursing in elderly patients with hip fracture,from database inception to January 31st,2026.The Cochrane Risk of Bias tool and the Newcastle-Ottawa Scale were used to assess the quality of randomized controlled trails and cohort studies,respectively.Meta-analysis was performed using R 4.2.3 software.Results A total of 25 studies involving 2 157 elderly patients with hip fracture were included.Meta-analysis results showed that the refined nursing group had significantly higher clinical effectiveness(OR=5.52,95%CI:3.14-9.72,P＜0.0001)and nursing satisfaction(OR=3.36,95%CI:2.32-4.84,P＜0.0001)compared to the routine nursing group.The incidence of complications(OR=0.21,95%CI:0.13-0.32,P＜0.0001)and visual analogue scale pain scores(MD=-2.78,95%CI:-3.46--2.11,P＜0.001)were significantly lower in the refined nursing group.Conclusion Refined nursing can significantly improve postoperative rehabilitation outcomes in elderly patients with hip fracture.It is superior to routine nursing in enhancing clinical effectiveness and nursing satisfaction,reducing the incidence of complications,alleviating postoperative pain,demonstrating high value for clinical application and promotion.
