六西格玛管理在降低儿童龋齿治疗全身麻醉术后咽痛发生率中的应用OA
Application of Six Sigma management in reducing the incidence of postoperative sore throat in children with severe dental caries under general anesthesia
目的 探讨六西格玛核心方法定义、测量、分析、改进和控制(DMAIC)在降低严重儿童龋齿治疗全身麻醉后咽痛发生率中的应用效果.方法 选择2025年1~4月空军军医大学第三附属医院麻醉科口腔舒适化治疗中心接受气管插管全身麻醉下儿童龋齿治疗的患儿300例为研究对象,美国麻醉医师协会分级Ⅰ或Ⅱ级,年龄2~6岁,采用前后对照研究设计,按时间先后顺序将2025年1~2 月患儿设为改进前组(对照组,n=150),2025年3~4月患儿设为改进后组(观察组,n=150);比较改进前后两组患儿术后2h及术后24h咽痛发生率、谵妄发生率及家长、口腔医师满意度等指标.结果 经过改进,对照组与观察组患儿术后2h咽痛总发生率从33.3%下降至18.7%(P<0.05);术后24h咽痛总发生率从32.0%下降至7.3%(P<0.001).同时术后谵妄发生率从54.7%下降至36.7%(P<0.05);家长总体满意度显著高于对照组(94.0%vs 86.0%,P<0.05),差异有统计意义.结论 基于DMAIC方法优化麻醉管理与术后恢复流程,能有效降低儿童全身麻醉术后2、24h咽痛发生率及术后谵妄发生率,提高患儿恢复质量.
Objective To investigate the effect of the core method define,measure,analyze,improve,and control(DMAIC)of Six Sigma in reducing the incidence of postoperative sore throat in children with severe dental caries under general anesthesia.Methods A total of 300 children,aged 2-6 years,of American Society of Anesthesiologists(ASA)physical statusⅠ or Ⅱ,who underwent dental caries treatment under general anesthesia with tracheal intubation in the Dental Anesthesia Center of the Anesthesiology Department of the Third Affiliated Hospital of Air Force Medical University from January to April 2025 were selected as the research subjects.The relevant data were sorted by time series.The research subjects were divided into the control group,children receiving treatments from January to February 2025 were allocated into the pre-improvement group(n=150),and the observation group,those receiving treatments from March to April 2025 were allocated into the post-improvement group(n=150)according to the DMAIC core method of Six Sigma.The incidence of sore throat at 2h and 24h after surgery,the incidence of delirium,and the satisfaction of family members and oral doctors were compared between the two groups.Results After improvement,compared with the control group,the incidence of sore throat of the observation group at 2h after surgery decreased from 33.3%to 18.7%(P<0.05),and the incidence of sore throat at 24h after surgery decreased from 32.0%to 7.3%(P<0.001).At the same time,compared with the control group,the incidence of postoperative delirium of the observation group decreased from 54.7%to 36.7%(P<0.05),the overall satisfaction of family members was significantly higher than that of the control group(94.0%vs 86.0%,P<0.05),with a statistically significant difference.Conclusion The application of DMAIC in optimizing anesthesia management and postoperative recovery processes can effectively reduce the incidence of sore throat at 2h and 24h in children after general anesthesia,as well as the incidence of postoperative delirium,thereby improving the quality of recovery in children.
