The Department of Anesthesiology is characterized by the dual features of high risk and technological intensity.Adverse events occur frequently,posing a direct threat to patient safety.Establishing a systematic,non-punitive adverse event debriefing mechanism is essential for identifying systemic vulnerabilities,facilitating the translation of case-specific lessons into clinical improvements,and optimizing clinical workflows.This paper first reviews the evolution of the concept of anesthesia-related adverse events and analyzes the major limitations of existing reporting systems,as well as specialty-specific improvement strategies.It then describes the overall architecture and standardized procedures of the debriefing mechanism,including organizational design,a six-step closed-loop management process,and information technology support.Key practical tools are highlighted,such as root cause analysis,human factors theory,failure mode and effects analysis,and simulation,based debriefing.Case studies based on typical clinical scenarios,including respiratory and circulatory abnormalities,medication errors,and equipment failures,are provided.The paper also discusses the cultural,resource-related,and procedural challenges encountered during implementation.Finally,an effective evaluation indicator system is constructed,and future development directions are discussed.This paper aims to provide theoretical support and practical guidance for establishing an effective debriefing mechanism in the Department of Anesthesiology.
