Adverse event reporting is a fundamental part of medical quality and patient safety governance.Its value lies not only in documenting what has already happened,but also in identifying risks early,promoting organizational learning,and supporting continuous improvement.In many healthcare institutions,however,the system has yet to move beyond a predominantly documentation-and archiving-oriented mode of operation,and still falls short of functioning as a truly risk-based learning tool.The most prominent problems at present are not simply insufficient reporting volume,but rather varying degrees of deviation in what is reported,what is prioritized for analysis,and how the system is managed.In practice,complications are more likely to enter the field of reporting and discussion,whereas preventable adverse events are less readily recognized;events that have already caused harm tend to receive greater attention,whereas unsafe conditions and near misses are often overlooked;non-punitive reporting has been written into institutional policies,yet the motivation for voluntary reporting remains unstable;and although reporting platforms have gradually improved,analysis,feedback,and continuous improvement have not kept pace.These patterns suggest that,in some institutions,adverse event reporting still functions mainly as a mechanism for documentation and archiving,and its role as a tool for patient safety learning has not been fully realized.Against this background,and drawing on current policy requirements as well as recent research,this article reviews the major problems in the implementation of adverse event reporting systems from the perspectives of system orientation,classification,cultural mechanisms,system design,and closed-loop management,and proposes corresponding directions for improvement.
