目的 探讨磁共振表观弥散系数(ADC)值与直肠癌患者分化程度及血小板-内皮细胞黏附分子-1(CD31)、人表皮生长因子受体2(HER2)表达水平的相关性.方法 选取2022年1月—2025年1月山西医科大学附属运城市中心医院收治的104例直肠癌患者,根据病理结果分为中高分化组(n=65)与低分化组(n=39).比较两组术前ADC值,绘制受试者工作特征曲线分析ADC值对分化程度的评估价值.采用免疫组织化学法检测两组癌组织CD31、HER2表达阳性率,Spearman相关性分析ADC值与CD31、HER2表达的相关性.采用多因素Logistic回归分析直肠癌患者分化程度的影响因素.结果 低分化组ADC值低于中高分化组,癌组织CD31、HER2表达阳性率均高于中高分化组(P<0.05).ADC值评估直肠癌患者分化程度的曲线下面积为0.913,特异度与敏感度分别为85.49%、86.75%.直肠癌患者ADC值与CD31、HER2表达呈负相关(P<0.01).吸烟史、CD31表达、HER2表达、ADC值均为直肠癌患者分化程度的独立影响因素(P<0.05).结论 ADC值在评估直肠癌分化程度方面存在较高的价值,且与CD31、HER2表达水平呈负相关.ADC值、CD31表达、HER2表达以及吸烟史均是直肠癌低分化的独立影响因素.

Objective To explore the correlation of the apparent diffusion coefficient(ADC)value of magnetic resonance imaging(MRI)with the differentiation degree and the expression levels of platelet endothelial cell adhesion molecule-1(CD31)and human epidermal growth factor receptor 2(HER2)in rectal cancer.Methods A total of 104 rectal cancer patients admitted to Yuncheng Central Hospital Affiliated to Shanxi Medical University from January 2022 to January 2025 were selected.According to the pathological results,they were divided into the moderately/well-differentiated group(n=65)and the poorly differentiated group(n=39).The preoperative ADC values of the two groups were compared,and the receiver operating characteristic(ROC)curve was drawn to analyze the evaluation value of ADC values for the degree of differentiation.The positive expression rates of CD31 and HER2 in the cancer tissues of the two groups were detected by immunohistochemistry,and the correlation between ADC values and the expression of CD31 and HER2 was analyzed by Spearman correlation.Multivariate logistic regression was used to analyze the influencing factors of the degree of differentiation in rectal cancer patients.Results The ADC value of the poorly differentiated group was lower than that of the moderately/well-differentiated group,while the positive expression rates of CD31 and HER2 in the cancer tissues were both higher than those of the moderately/well-differentiated group(P<0.05).The area under the ROC curve of ADC value for evaluating the degree of differentiation of rectal cancer patients was 0.913,with a specificity and a sensitivity of 85.49%and 86.75%,respectively.The ADC value of rectal cancer patients was negatively correlated with the expression of CD31 and HER2(P<0.01).Smoking history,CD31 expression,HER2 expression,and ADC value were all independent influencing factors for the degree of differentiation of rectal cancer patients(P<0.05).Conclusion The ADC value has a high value in assessing the degree of differentiation of rectal cancer,and it is negatively correlated with the expression level of CD31 and HER2.ADC value,CD31 expression,HER2 expression,and smoking history are all independent influencing factors for the low differentiation of rectal cancer.
