c-Myc与Bcl-2基因表达在新诊断多发性骨髓瘤患者疗效及预后评估中的临床意义OA
Clinical significance of c-Myc and Bcl-2 gene expression for response and prognosis evaluation in patients with newly diagnosed multiple myeloma
目的 探讨新诊断多发性骨髓瘤(NDMM)患者骨髓c-Myc、Bcl-2基因表达与临床特征、疗效及预后的关系.方法 回顾性分析解放军联勤保障部队第九八〇医院2020年1月—2024年12月收治的60例NDMM患者的临床资料,包括c-Myc、Bcl-2基因均表达的研究组(n=20)和二者均不表达的对照组(n=40).两组均接受完整的VRd方案(硼替佐米、来那度胺及地塞米松)化疗.比较两组一般临床特征及疗效,采用GraphPad Prism 11.0软件进行Kaplan-Meier生存分析,采用多因素Cox回归分析影响总生存期(OS)和无进展生存期(PFS)的相关因素.随访结束后动态分析NDMM患者c-Myc、Bcl-2基因表达情况.结果 研究组贫血、肾功能异常占比以及乳酸脱氢酶、β2-微球蛋白水平均高于对照组(P<0.05).两组VRd方案化疗后总缓解率比较差异无统计学意义(P>0.05).研究组中位OS(14.7个月vs.21.6个月)及PFS(7.6个月vs.13.9个月)较对照组短(P<0.05).贫血是影响NDMM患者OS的独立不良预后因素(P<0.05).随访4.8~56.0个月后,研究组12例(60%)c-Myc、Bcl-2基因表达转阴(均不表达)患者中,6例(50%)出现基因复阳(均表达)并伴有临床复发.结论 c-Myc和Bcl-2基因表达与NDMM更差的临床特征、更低的治疗应答和更高的复发风险相关,动态监测显示二者基因表达在缓解时转阴、复发时复阳,提示二者可能是驱动疾病进展和复发的关键因子.
Objective To investigate the relationship of the expression of c-Myc and Bcl-2 genes in the bone marrow of patients with newly diagnosed multiple myeloma(NDMM)with their clinical characteristics,therapeutic efficacy,and prognosis.Methods A retrospective analysis was conducted on the clinical data of 60 patients with NDMM admitted to the 980th Hospital of the Joint Logistic Support Force of the PLA from January 2020 to December 2024.The study group included 20 patients with both c-Myc and Bcl-2 genes expressed,while the control group consisted of 40 patients with neither gene expressed.Both groups received the standard Bortezomib,Lenalidomide,and Dexamethasone(VRd)regimen.The general clinical characteristics and therapeutic efficacy of the two groups were compared.Kaplan-Meier survival analysis was performed using GraphPad Prism 11.0 software,and multivariate Cox regression analysis was used to identify the factors influencing overall survival(OS)and progression-free survival(PFS).The expression of c-Myc and Bcl-2 genes in NDMM patients was dynamically analyzed after the follow-up period.Results The proportions of anemia and abnormal renal function in the study group,as well as the levels of lactate dehydrogenase and β2-microglobulin,were all higher than those in the control group(P<0.05).There was no significant difference in the total response rate after treatment with VRd regimen between the two groups(P>0.05).The median OS(14.7 months vs.21.6 months)and PFS(7.6 months vs.13.9 months)of the study group were shorter than those of the control group(P<0.05).Anemia was an independent adverse prognostic factor for OS in patients with NDMM(P<0.05).After a follow-up period of 4.8 to 56.0 months,among the 12 patients(60%)in the study group whose c-Myc and Bcl-2 gene expressions became negative(negative for both),6(50%)showed re-expression of both genes(positive for both),accompanied by clinical recurrence.Conclusion The expression of c-Myc and Bcl-2 genes is associated with more unfavorable clinical characteristics,lower treatment response,and higher recurrence risk in patients with NDMM.Dynamic monitoring shows that the expression of these two genes becomes negative during remission and reappears at recurrence,suggesting that they may be key molecules driving disease progression and recurrence.
