目的 探讨基于超声评估不同美国放射学会甲状腺成像报告数据系统(ACR-TIRADS)分类甲状腺结节的体积与直径自然变化特点及其临床意义.方法 采用回顾性队列研究,纳入2018年1月—2025年9月每季度第1个月在解放军总医院第一医学中心就诊并完成超声随访的5 963例甲状腺结节患者,共13 815枚结节,分析基于体积/直径评估标准显著增大结节的检出率、自然生长规律及观察期内恶性检出率.结果 平均随访时长(49.2±4.8)个月,基线时患者平均年龄(49.9±13.4)岁,女性占比74.0%(4 410/5 963),83.7%(4 993/5 963)的患者为多发结节.随访期间,25.6%(3 538/13 815)的结节显著增大,61.4%(8 485/13 815)保持稳定状态,11.1%(1 533/13 815)明显缩小,1.9%(259/13 815)完全消失.进一步分析显示,基于体积评估标准(VBC)显著增大结节的检出率高于基于直径评估标准(DBC)的检出率,分别为24.7%(3 409/13 815)和22.9%(3 164/13 815),P<0.05.在49个月的观察期内,不同ACR-TIRADS分类且显著增大结节均呈现"慢-快-慢"的生长模式,其中ACR-TIRADS 1~3类结节增长高峰期在随访期第24~36个月,而4、5类结节增长高峰期则在随访期第12~24个月.此外,随访期间共有577枚结节(稳定组324枚和显著增大组253枚)接受细针穿刺活检,显著增大组甲状腺乳头状癌检出率显著高于稳定组,分别为25.3%(64/253)和6.5%(21/324),而显著增大组中分别基于VBC/DBC的检出率相当,分别为25.4%(60/236)和26.0%(57/219).结论 VBC较DBC对显著增大结节的检出灵敏度更高,但二者甲状腺乳头状癌检出效能相当;不同ACR-TI-RADS风险结节的生长模式存在时间异质性,可为临床个体化随访策略制定提供循证参考.

Objective To investigate the natural change characteristics of the volume and diameter of thyroid nodules stratified by different American College of Radiology Thyroid Imaging Reporting and Data System(ACR-TIRADS)categories assessed by ultrasound,as well as their clinical significance.Methods A retrospec-tive cohort study was conducted.A total of 5 963 patients with thyroid nodules(13 815 nodules in total)who visited The First Medical Center of Chinese PLA General Hospital in the first month of each quarter from January 2018 to September 2025 and completed an ultrasound follow-up were enrolled.The detection rate of significantly enlarged nodules,their natural growth patterns,and the malignancy detection rate were ana-lyzed under the volume-based criterion(VBC)and diameter-based criterion(DBC).Results The mean fol-low-up duration was(49.2±4.8)months.At baseline,the mean age of patients was(49.9±13.4)years,with fe-males accounting for 74.0%(4 410/5 963),and 83.7%(4 993/5 963)of patients having multiple nodules.During follow-up,25.6%(3 538/13 815)of the nodules were significantly enlarged,61.4%(8 485/13 815)re-mained stable,11.1%(1 533/13 815)shrank significantly,and 1.9%(259/13 815)resolved completely.Further analysis showed that the detection rate of significantly enlarged nodules was higher under VBC than under DBC,with 24.7%(3 409/13 815)vs 22.9%(3 164/13 815),respectively(P<0.05).During the 49-month ob-servation period,all significantly enlarged nodules with different ACR-TIRADS categories presented a"slow-fast-slow"growth pattern.The peak growth period of nodules in ACR-TIRADS categories 1~3 occurred at 24~36 months of follow-up,while that of nodules in ACR-TIRADS categories 4 and 5 occurred at 12~24 months.In addition,a total of 577 nodules(324 in the stable group and 253 in the significantly enlarged group)under-went fine-needle aspiration biopsy during follow-up.The detection rate of papillary thyroid carcinoma in the significantly enlarged group was much higher than that in the stable group,at 25.3%(64/253)vs 6.5%(21/324),respectively.Among the significantly enlarged nodules,the papillary thyroid carcinoma detection rates under VBC and DBC were comparable,at 25.4%(60/236)vs 26.0%(57/219),respectively.Conclusions VBC has a higher detection sensitivity for significantly enlarged nodules than DBC,while the papillary thyroid car-cinoma detection efficacy is comparable between the two criteria.Temporal heterogeneity exists in the growth patterns of nodules with different ACR-TIRADS risk stratifications,which can provide evidence-based refer-ence for the development of individualized clinical follow-up strategies.
