目的 探讨初诊Graves病(GD)患者在抗甲状腺药物(ATD)治疗期间甲状腺体积的动态变化特征及其与治疗反应的关系.方法 回顾性分析基线及ATD治疗后第3、6个月的甲状腺功能、促甲状腺激素受体抗体(TRAb)水平及甲状腺体积等数据.采用潜类增长模型(LCGM)识别甲状腺体积的变化轨迹,并分析ATD疗效的独立影响因素.结果 基线甲状腺体积与游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)、TRAb、GREAT评分及FT4达标时间均呈正相关(P<0.05).治疗期间甲状腺体积的降幅与FT4及TRAb的降幅呈正相关,与基线GREAT评分呈负相关(P<0.05).LCGM可识别出低水平与高水平两种体积变化轨迹组.与高水平组相比,低水平组基线FT3、FT4、TRAb及GREAT评分更低,FT4及甲状腺功能达标时间更短(P<0.05).低水平体积轨迹是第3、6个月FT4达标的独立影响因素(OR=3.92,95%CI:[1.40,11.28];OR=8.82,95%CI:[1.61,65.73]).结论 临床实践中定期监测GD患者甲状腺体积变化,有助于评估ATD治疗效果.

Objective To investigate the dynamic changes of thyroid volume and their relationship with treat-ment response in newly diagnosed adult patients with Graves'disease(GD)undergoing antithyroid drug(ATD)therapy.Methods A retrospective analysis was conducted on thyroid function,thyrotropin receptor antibody(TRAb)levels,and thyroid volume at baseline and at 3 and 6 months of ATD treatment.Latent class growth modeling(LCGM)was used to identify thyroid volume trajectories,and independent factors asso-ciated with ATD efficacy were analyzed.Results Baseline thyroid volume was positively correlated with free triiodothyronine(FT3),free thyroxine(FT4),TRAb levels,GREAT score,and time to FT4 normalization(P<0.05).The reduction in thyroid volume during treatment was positively correlated with decreases in FT4 and TRAb,but negatively correlated with baseline GREAT score(P<0.05).LCGM identified two distinct volume trajectories.Compared with the high-level trajectory group,the low-level group had lower baseline FT3,FT4,TRAb,and GREAT scores,as well as shorter time to achieve FT4 and overall thyroid function normalization(P<0.05).The low-level trajectory was an independent factor for FT4 normalization at 3 and 6 months(OR=3.92,95%CI:[1.40,11.28];OR=8.82,95%CI:[1.61,65.73]).Conclusion Regular monitoring of thyroid volume changes during ATD therapy is clinically valuable for evaluating treatment response.
