血清FGF21、ANXA1、KLF2水平与下肢动脉硬化闭塞症患者支架植入术后1年内发生支架内再狭窄的关系OA
Relationship between serum FGF21,ANXA1 and KLF2 levels and in-stent restenosis within 1 year after stent implantation in patients with lower extremity arteriosclerosis obliterans
目的 探讨血清成纤维细胞生长因子21(FGF21)、膜联蛋白 A1(ANXA1)、Krüppel样转录因子(KLF)2水平与下肢动脉硬化闭塞症(LEASO)患者支架植入术后1年内发生支架内再狭窄(ISR)的关系.方法 选取2021年8月至2024年8月该院收治的183例LEASO患者作为研究对象,均接受支架植入术治疗,根据术后1年是否发生ISR,将患者分为ISR组和 N-ISR组.采用多因素Logistic回归分析LEASO患者支架植入术后1年内发生ISR的影响因素.采用受试者工作特征(ROC)曲线分析血清FGF21、ANXA1、KLF2单独及3项联合对LEASO患者支架植入术后1年内发生ISR的预测效能.通过校准曲线、Hosmer-Leme-show检验联合决策分析曲线对联合预测模型进行内部验证和临床适用性分析.结果 ISR组血清FGF21、ANXA1水平及有吸烟史、高血压、糖尿病患者比例均高于N-ISR组,血清KLF2水平低于 N-ISR组,差异均有统计学意义(P<0.05).多因素Logistic回归分析结果显示,有吸烟史、合并糖尿病,以及血清FGF21、ANXA1水平升高是 LEASO 患者支架植入术后1年内发生ISR 的危险因素(P<0.05),血清 KLF2水平升高是LEASO患者支架植入术后1年内发生ISR的保护因素(P<0.05).ROC曲线分析结果显示,血清FGF21、ANXA1、KLF2联合预测LEASO患者支架植入术后1年内发生ISR的曲线下面积(AUC)为0.918,大于血清FGF21、ANXA1、KLF2单独预测的 AUC(Z联合-FGF21=2.650,P联合-FGF21=0.008;Z联合-ANXA1=2.856,P联合-ANXA1=0.004;Z联合-KLF2=3.223,P联合-KLF2=0.001).联合预测模型的校准曲线与理想曲线一致性良好,Hosmer-Leme-show检验χ2=6.315,P=0.612.在高风险阈值0.08~0.96内,血清FGF21、ANXA1、KLF2联合模型预测LEASO患者支架植入术后1年内发生ISR的净获益率优于单一指标预测.结论 血清 FGF21、ANXA1、KLF2水平与LEASO患者支架植入术后1年内ISR发生有关,且对LEASO患者支架植入术后ISR发生具有一定的预测价值和临床适用性.
Objective To investigate the relationship between serum fibroblast growth factor 21(FGF21),annexin A1(ANXA1)and Krüppel-like transcription factor(KLF)2 levels and in-stent restenosis(ISR)within 1 year after stent implantation in patients with lower extremity arteriosclerosis obliterans(LEASO).Methods A total of 183 LEASO patients admitted to the hospital from August 2021 to August 2024 were se-lected as the research objects.All patients received stent implantation.Multivariate Logistic regression was used to analyze the influencing factors of ISR within 1 year after stent implantation in LEASO patients.The receiver operating characteristic(ROC)curve was used to analyze the predictive efficacy of serum FGF21,ANXA1 and KLF2 alone and in combination for ISR within 1 year after stent implantation in LEASO pa-tients.The calibration curve and Hosmer-Lemeshow test combined decision analysis curve were used for inter-nal validation and clinical applicability analysis of the combined prediction model.Results The levels of serum FGF21 and ANXA1,as well as the proportions of patients with smoking history,hypertension and diabetes in the ISR group were all higher than those in the N-ISR group,while the level of serum KLF2 in the ISR group was lower than that in the N-ISR group,and the differences were statistically significant(P<0.05).Multiva-riate Logistic regression analysis showed that smoking history,diabetes,and elevated serum FGF21 and ANXA1 levels were risk factors for ISR within 1 year after stent implantation in LEASO patients(P<0.05).Increased serum KLF2 level was a protective factor for ISR within 1 year after stent implantation in LEASO patients(P<0.05).The results of ROC curve analysis showed that the area under the curve(AUC)of serum FGF21,ANXA1 and KLF2 combined to predict ISR within 1 year after stent implantation in LEASO patients was 0.918,which was greater than the AUC of serum FGF21,ANXA1 and KLF2 alone(Zcombined-FGF21=2.650;Pcombined-FGF21=0.008;Zcombination-ANXA1=2.856,Pcombination-ANXA1=0.004;Zcombination-KLF2=3.223,Pcombination-KLF2=0.001).The calibration curve of the combined prediction model was in good agreement with the ideal curve(Hosmer-Lemeshow test χ2=6.315,P=0.612).Within the high-risk threshold of 0.08-0.96,the net bene-fit rate of serum FGF21,ANXA1 and KLF2 combined model in predicting ISR within 1 year after stent im-plantation in LEASO patients was better than that of the single index.Conclusion The serum levels of FGF21,ANXA1 and KLF2 are related to the occurrence of ISR within 1 year after stent implantation in LEASO patients,and they have certain predictive value and clinical applicability for ISR in LEASO patients af-ter stent implantation.
