目的 探讨首发精神分裂症患者血清激肽释放酶6(KLK6)、铁调素、磷酸酶张力蛋白同源物(PTEN)与病情严重程度及认知功能受损的关系.方法 选取2023年7月至2025年3月该院收治的121例首发精神分裂症患者纳入精神分裂症组,另选取同期在该院进行健康体检的121例无精神疾病的体检者纳入对照组.根据阳性与阴性症状量表(PANSS)评分,将患者分为轻度组(60分≤PANSS评分<75分)、中度组(75分≤PANSS评分<95分)和重度组(PANSS≥95分).根据 MATRIS成套神经认知测试(MCCB)评分,将患者分为认知功能受损组(MCCB评分<40分)和认知功能正常组(MCCB评分≥40分).采用酶联免疫吸附试验检测患者血清KLK6、铁调素、PTEN水平.采用多因素Logistic回归分析首发精神分裂症患者认知功能受损的影响因素.采用受试者工作特征(ROC)曲线分析血清KLK6、铁调素、PTEN对首发精神分裂症患者发生认知功能受损的预测价值.结果 精神分裂症组血清 KLK6、铁调素水平高于对照组,PTEN水平低于对照组,差异均有统计学意义(P<0.05).重度组血清 KLK6、铁调素水平高于轻、中度组,且中度组血清 KLK6、铁调素水平高于轻度组,差异均有统计学意义(P<0.05).重度组血清PTEN水平低于轻、中度组,且中度组血清PTEN水平低于轻度组,差异均有统计学意义(P<0.05).认知功能受损组血清 KLK6、铁调素水平及PANSS评分高于认知功能正常组,血清PTEN 水平低于认知功能正常组,差异均有统计学意义(P<0.05).多因素 Logistic回归分析结果显示,PANSS评分及血清KLK6、铁调素水平升高为首发精神分裂症患者认知功能受损的危险因素(P<0.05),血清PTEN 水平升高为首发精神分裂症患者认知功能受损的保护因素(P<0.05).ROC曲线分析结果显示,血清KLK6、铁调素、PTEN联合预测首发精神分裂症患者发生认知功能受损的曲线下面积(AUC)为0.953,大于血清 KLK6、铁调素、PTEN单独预测的 AUC(Z=2.596、2.751、2.493,均P<0.05).结论 首发精神分裂症患者血清 KLK6、铁调素水平升高,血清PTEN水平降低,3项指标水平与病情严重程度及认知功能受损有关,3项指标联合检测对首发精神分裂症患者发生认知功能受损的预测价值较高.

Objective To investigate the relationship between serum kallikrein 6(KLK6),hepcidin,phos-phatase and tensin homolog(PTEN),and disease severity and cognitive impairment in patients with first-epi-sode schizophrenia.Methods A total of 121 patients with first-episode schizophrenia admitted to the hospital from July 2023 to March 2025 were selected as the schizophrenia group,and 121 people without mental disor-ders who underwent physical examination in the hospital during the same period were selected as the control group.According to the Positive and Negative Syndrome Scale(PANSS)score,the patients were divided into the mild group(60 points≤PANSS score<75 points),the moderate group(75 points≤PANSS score<95 points)and the severe group(PANSS≥95 points).According to the MATRIS neurocognitive test(MCCB)score,the patients were divided into the cognitive impairment group(MCCB score<40 points)and the nor-mal cognitive function group(MCCB score≥40 points).The serum levels of KLK6,hepcidin and PTEN were detected by enzyme-linked immunosorbent assay.Multivariate Logistic regression was used to analyze the in-fluencing factors of cognitive impairment in first-episode schizophrenia.Receiver operating characteristic(ROC)curve was used to analyze the predictive value of serum KLK6,hepcidin and PTEN for cognitive im-pairment in patients with first-episode schizophrenia.Results The serum levels of KLK6 and hepcidin in the schizophrenia group were higher than those in the control group,and the level of PTEN was lower than that in the control group,and the differences were statistically significant(P<0.05).The levels of serum KLK6 and hepcidin in the severe group were higher than those in the mild and moderate groups,and the levels of serum KLK6 and hepcidin in the moderate group were higher than those in the mild group,and the differences were statistically significant(P<0.05).The serum PTEN level in the severe group was lower than that in the mild and moderate groups,and the serum PTEN level in the moderate group was lower than that in the mild group,and the differences were statistically significant(P<0.05).The serum levels of KLK6,hepcidin and PANSS score in the cognitive impairment group were higher than those in the normal cognitive function group,and the serum level of PTEN was lower than that in the normal cognitive function group,and the differences were statistically significant(P<0.05).Multivariate Logistic regression analysis showed that PANSS score,increased serum KLK6 and hepcidin levels were risk factors for cognitive impairment in patients with first-episode schizophrenia(P<0.05),and increased serum PTEN level was a protective factor for cogni-tive impairment in patients with first-episode schizophrenia(P<0.05).ROC curve analysis showed that the area under the curve(AUC)of serum KLK6,hepcidin and PTEN combined to predict cognitive impairment in patients with first-episode schizophrenia was 0.953,which was greater than the AUC of serum KLK6,hepci-din and PTEN alone(Z=2.596,2.751,2.493;all P<0.05).Conclusion The serum levels of KLK6 and hep-cidin are increased and the serum level of PTEN is decreased in patients with first-episode schizophrenia.The levels of the three indexes are related to the severity of the disease and cognitive impairment.The combined detection of the three indexes has a higher predictive value for cognitive impairment in patients with first-epi-sode schizophrenia.
