目的:回顾性总结分析消瘤治萎方对慢性萎缩性胃炎(CAG)的临床疗效,同时初步探究其作用机制.方法:收集2022 年 12 月—2024 年 12 月江西中医药大学附属医院脾胃肝胆科诊治的 CAG 患者临床资料,根据治疗方法的不同分为观察组(消瘤治萎方)和对照组(常规西药).分析和比较 2 组胃镜评分、病理(萎缩、肠上皮化生、上皮内瘤变)和中医证候积分及疗效,以及临床疗效等.同时综合运用网络药理学、生物信息学及分子对接技术,对其作用机制进行初步探索.结果:回顾性分析结果显示,观察组治疗后的胃镜评分、病理积分及中医证候评分优于对照组,差异均有统计学意义(P<0.01);同时,2 组各项观察指标均较治疗前有所改善(P<0.05 或 P<0.01).观察组在胃镜、病理、中医证候疗效及临床疗效分别达到 71.20%、80.30%、83.33%和 66.67%,均高于对照组,差异均有统计学意义(P<0.01).通过网络药理学分析筛选出 118个 CAG"炎—癌"与消瘤治萎方的共有靶点,基因本体和京都基因与基因组百科全书富集分析显示PI3K-PKB/Akt、IL-17、FoxO、TNF及p53 信号通路中均存在靶点富集,Cytoscape 相关模块进一步鉴定出 22 个核心基因.分子对接结果显示,槲皮素与潜在核心基因相互作用.结论:消瘤治萎方对 CAG 的疗效优于常规西药,网络药理学分析提示消瘤治萎方可能通过调控PI3K-PKB/Akt、IL-17、FoxO、TNF、p53 信号通路治疗CAG,其结果有待进一步实验验证.

Objective:To retrospectively summarize and analyze the clinical efficacy of Xiaoliu Zhiwei Formula(XLZWF)in treating chronic atrophic gastritis(CAG)and to preliminarily explore its mechanism of action.Methods:Clinical data of CAG patients from the Department of Spleen,Stomach,Liver,and Gallbladder of the Affiliated Hospital of Jiangxi University of Chinese Medicine between December 2022 and December 2024 were collected.Patients were divided into an experimental group(XLZWF)and a control group(conventional western medicine)based on different treatment methods.The gastroscope,pathology(atrophy,intestinal metaplasia,intraepithelial neoplasia),traditional Chinese medicine(TCM)syndrome score,and clinical efficacy were analyzed in both groups.Network pharmacology,bioinformatics,and molecular docking techniques were also used for preliminary exploration of its mechanism.Results:The retrospective analysis showed that after treatment,the gastroscope score,pathology score,and syndrome score in the experimental group were significantly better than those in the control group(all P<0.01).Additionally,various observation indicators improved in both groups compared to before treatment(P<0.05 or P<0.01).The total effective rates for gastroscope efficacy,pathological efficacy,syndrome efficacy,and clinical efficacy in the experimental group were 71.20%,80.30%,83.33%,and 66.67%,respectively,which were significantly higher than those in the control group(all P<0.01).Through network pharmacology analysis,118 common targets between inflammation-to-cancer transformation of CAG and XLZWF were identified.GO and KEGG enrichment analysis showed that targets were enriched in the PI3K-PKB/Akt,IL-17,FoxO,TNF,and p53 signaling pathways,and Cytoscape-related modules further identified 22 core genes.Molecular docking showed interactions between quercetin and potential core genes.Conclusion:This study preliminarily confirmed that XLZWF is superior to traditional western medicine in CAG patients.Network pharmacology analysis suggests that XLZWF may treat CAG by regulating the PI3K-PKB/Akt,IL-17,FoxO,TNF,and p53 signaling pathways,and these findings require further experimental verification.
