基于"络病理论"探讨虫类药在癌性疼痛中的应用OA

Malignant tumors commonly arise from the long-term accumulation of pathogenic toxins and blood stasis,and cancer pain is closely linked to such pathological changes.During tumor progression,stagnation of qi movement and impeded blood circulation lead to the internal generation of phlegm-dampness and the accumulation of heat-toxin.These pathogenic factors combine to obstruct the meridi-ans and collaterals,causing blockage of qi and blood flow,depriving the collateral vessels of patency,and ultimately inducing pain.As the tumor persists over time,healthy qi becomes depleted,leading to deficiency of healthy qi,malnutrition of the collaterals,and the de-velopment of collateral vessel pathology.This aligns with the characteristic noted in Case Records as a Guide to Clinical Practice that"prolonged illness and chronic pain inevitably involve the blood collateral system."Therefore,treatment follows the principle that"swift and penetrating agents are indispensable for efficacy"necessitating the use of insect drugs to expel wind,unblock collaterals,and relieve pain.Insect drugs,classified as agents derived from sentient beings with flesh and blood,possess potent and vigorous properties,charac-terized by their ability to ascend,disperse,and penetrate.Their therapeutic efficacy in dredging collateral vessels and dispelling wind pathogen far exceeds that of herbal and mineral medicines.Commonly prescribed agents include Wugong(Scolopendra),Quanxie(Scor-pio),Dilong(Pheretima),Tubiechong(Eupolyphaga seu Steleophaga),Shuizhi(Hirudo),etc.In clinical application,these drugs should be administered based on syndrome differentiation and treatment,with gradual dose escalation adjusted according to the patient's response,aiming to achieve the therapeutic objective of"eliminating pathogens without damaging healthy qi,and achieving rapid effi-cacy without fierce or harmful effects."
