中药调控线粒体自噬防治血管性痴呆研究进展OA

血管性痴呆(vascular dementia,VD)是脑血管疾病所致的慢性认知障碍综合征,核心机制包括慢性脑低灌注、氧化应激、神经炎症和神经元凋亡等.线粒体自噬作为线粒体质量控制的核心机制,在 VD 的发病及防治中发挥着关键作用.中药具有多靶点、双向调节及整体观念的独特优势,可通过激活 PINK1/Parkin、HIF-1α/BNIP3 及 PI3K/AKT/mTOR 等信号通路,精准调控线粒体自噬,有效清除细胞内受损线粒体,从而减轻氧化应激、抑制炎症反应、减少细胞凋亡,进而发挥神经保护作用,改善学习与记忆功能.延龄草总皂苷、槲皮素、肉豆蔻木脂素等中药有效单体,当归补血汤、滋肾活血方、通脉开窍丸、定志小丸等经典复方,均已显示出良好的线粒体自噬调控活性.但现有研究仍存在一定局限性:(1)多数研究仅停留在蛋白表达相关性层面,缺乏对蛋白磷酸化等翻译后修饰动态过程的深入探讨;(2)临床研究相对匮乏,尚缺乏高质量的临床试验以验证中药调控线粒体自噬与VD 患者临床疗效之间的量效关系;(3)中药多成分协同调控多种信号通路的具体网络机制尚不完全明确.随着多学科交叉技术的融合与创新,未来有望进一步阐明并发挥中医药在 VD 防治中的独特优势,为应对人口老龄化带来的痴呆挑战提供更有效的中医药解决方案.

Vascular dementia(VD)is a chronic cognitive impairment syndrome induced by cerebrovascular diseases.Its core pathogen-ic mechanisms consist of chronic cerebral hypoperfusion,oxidative stress,neuroinflammation and neuronal apoptosis.As a core mecha-nism for mitochondrial quality control,mitophagy plays a vital role in the pathogenesis,prevention and treatment of VD.Chinese medici-nals possess unique superiorities including multi-target effects,bidirectional regulatory properties and holistic therapeutic concepts.Chi-nese medicinals can accurately modulate mitophagy by activating multiple signaling pathways such as PINK1/Parkin,HIF1α/BNIP3 and PI3K/AKT/mTOR.This regulatory effect promotes the clearance of intracellular damaged mitochondria,alleviates oxidative stress,inhibits neuroinflammation and reduces neuronal apoptosis,so as to exert neuroprotective effects and improve learning and memory func-tions.Active monomeric constituents of Chinese medicinals,including Trillium tschonoskii total saponins(TTTS),quercetin,and myris-ticin lignans,as well as some classical compound formulas such as Chinese Angelica Blood-Supplementing Decoction,Kidney-Nourishing and Blood-Activating Formula,Channel-Unblocking and Orifice-Opening Pills,Minor Dingzhi Pills,all exert favorable regulatory effects on mitophagy.However,existing research still has certain limitations:(1)Most existing studies only focus on static correlations of pro-tein expression,lacking in-depth research on dynamic post-translational modification events represented by protein phosphorylation;(2)Clinical research evidence remains insufficient,and high-quality clinical trials are still scarce to validate the dose-response relationship between mitophagy modulation mediated by Chinese medicinals and clinical therapeutic efficacy in patients with VD;(3)The precise network mechanisms underlying the synergistic modulation of multiple signaling pathways by multi-ingredient Chinese medicinals remain incompletely elucidated.Although the mechanistic resolution of existing research remains inadequate,the integration and innovative de-velopment of interdisciplinary technologies are expected to further decipher the underlying molecular mechanisms and exploit the unique strengths of TCM in the prevention and treatment of VD.This will provide more effective TCM therapeutic strategies to tackle the VD crisis driven by population aging.
