常见7种矿物质与2型糖尿病因果关联的孟德尔随机化研究OA
Mendelian randomization study on the causal associations between 7 com-mon minerals and type 2 diabetes mellitus
目的:探讨铜、硒、锌、钙、铁、镁和钾等7 种常见矿物质与 2 型糖尿病(T2DM)之间的因果关联.方法:基于全基因组关联研究相关数据库,以7 种矿物质的单核苷酸多态性(SNP)作为暴露,T2DM 为结局进行正向 MR分析;以 T2DM 的 SNP 作为暴露,7 种矿物质血液浓度显著相关的 SNP 为结局进行反向 MR 分析;以7 种矿物质血液浓度显著相关的 SNP 为暴露,以 T2DM 相关特征[糖化血红蛋白、空腹胰岛素、空腹血糖、稳态模型评估的胰岛素抵抗指数(HOMA-IR)、HOMA-β 和胰岛素受体]为结局进行 MR 分析.以逆方差加权法(IVW)为主要分析方法,同时使用 MR-Egger、加权中位数、简单模式和加权模式等 4 种方法进行辅助分析.采用 Cochran's Q 检验评估异质性、MR-Egger 截距法检验多效性、留一法进行敏感性分析.结果:正向 MR 分析 IVW 法发现硒可能增加 T2DM 的风险(OR=1.036,95%CI 为1.002~1.071),反向 MR 分析 IVW 法结果显示 T2DM 与硒无关联(OR=1.087,95%CI 为0.971~1.217),辅助方法与 IVW 法结果一致,预测结果稳健;未发现其他矿物质与 T2DM 存在关联(P>0.05).硒与空腹血糖(OR=1.025,95%CI 为 1.008~1.042)和胰岛素抵抗指数(OR=1.033,95%CI 为 1.011~1.057)均存在因果关联.结论:高水平硒与 T2DM 风险增加有关.
Aim:To investigate the causal associations between 7 common minerals(copper,selenium,zinc,calcium,i-ron,magnesium,and potassium)and the risk of type 2 diabetes mellitus(T2DM).Methods:Based on genome-wide associ-ation study databases,forward Mendelian randomization(MR)analysis was conducted using SNPs related with blood con-centration of 7 minerals as exposures and T2DM-related SNPs as the outcome;reverse MR analysis was performed with T2DM-related SNPs as exposures and the 7 minerals' SNPs as outcomes.MR analysis was conducted using SNPs of 7 miner-als as exposures and T2DM-related characteristics,including glycated hemoglobin,fasting insulin,fasting blood glucose,HO-MA-IR,HOMA-β,and insulin receptor,as outcomes.Inverse variance weighting(IVW)method was employed as the prima-ry analytical approach,supplemented by 4 additional methods(MR-Egger,weighted median,simple pattern,and weighted pattern).Heterogeneity was assessed using Cochran's Q test,heterogeneity was evaluated with the MR-Egger intercept test,and sensitivity analyses were conducted via the leave-one-out method.Results:Forward MR analysis using the IVW method revealed that selenium may increase the risk of T2DM(OR was 1.036,95%CI was 1.002 to 1.071),whereas reverse MR analysis with IVW showed no association between T2DM and selenium(OR was 1.087,95%CI was 0.971 to 1.217),and the supplementary methods yielded consistent results with the IVW approach,demonstrating robust predictive validity.No associations were found between other minerals and T2DM(P>0.05).Selenium exhibited causal relationships with fasting blood glucose(OR was 1.025,95%CI was 1.008 to 1.042)and insulin resistance index(OR was 1.033,95%CI was 1.011 to 1.057).Conclusion:Higher selenium levels are associated with an increased risk of T2DM.
