巴戟天丸在骨质疏松性骨折愈合中的机制研究OA
Mechanism of Baijitian Pill in Healing Osteoporotic Fracture
目的:为明确巴戟天丸干预骨质疏松性骨折的分子机制,本文探究 CD147 介导巨噬细胞调控破骨细胞功能在骨折修复中的作用.方法:选取50 只 SPF 级 SD 雌性大鼠,随机分为正常(N)组,模型(C)组,巴戟天丸低剂量(L)组,巴戟天丸中剂量(M)组,巴戟天丸高剂量(H)组,每组 10 只,N 组不建模,其余 4 组先去卵巢饲养 12 周建立骨质疏松模型,再行右股骨中段横断骨折并髓内针固定,完成骨质疏松症骨折建模,建模成功后,对 L 组、M 组、H 组给予巴戟天丸 0.4、0.8、1.2g/mL 灌胃,每天 1次,连续8 周,N 组、C 组同期灌胃同体积生理盐水,检查大鼠骨密度及骨生物力力学,取骨组织进行 HE染色,TRAP 染色检测破骨细胞生成,酶联免疫吸附法检测血清巨噬细胞相关因子水平,免疫印迹法检测骨组织 CD147、RANKL、OPG 蛋白表达.结果:与 N 组相比,C 组骨密度、最大载荷、弹性模量、断裂能量降低(P<0.05),与 C 组相比,L 组、M 组、H 组最大载荷、弹性模量、断裂能量升高(P<0.05),且 M 组比 L 组升高明显(P<0.05),H 组比 M 组升高明显(P<0.05);N 组骨小梁排列规则、致密,结构完整,骨基质均匀,无明显间隙,C 组骨痂组织中骨小梁稀疏、断裂,排列紊乱,与 C 组比较,L 组、M 组、H 组骨组织病理形态明显改善;与 N 组比较,C 组骨组织破骨细胞数量升高(P<0.05),与 C 组比较,L 组、M 组、H 组骨组织破骨细胞数量降低(P<0.05),且 M 组比 L 组降低明显(P<0.05),H 组比 M 组降低明显(P<0.05);与 N 组相比,C 组血清 TNF-α、IL-6 水平升高(P<0.05),IL-10、TGF-β 水平降低(P<0.05),与 C 组相比,L 组、M 组、H 组 TNF-α、IL-6 水平降低(P<0.05),IL-10、TGF-β 水平升高(P<0.05),且M 组比 L 组变化明显(P<0.05),H 组比 M 组变化明显(P<0.05);与 N 组相比,C 组骨组织 CD147、RANKL 蛋白表达升高(P<0.05),OPG 蛋白表达降低(P<0.05),与 C 组相比,L 组、M 组、H 组 CD147、RANKL 蛋白表达降低(P<0.05),OPG 蛋白表达升高(P<0.05),且 M 组比 L 组变化明显(P<0.05),H组比 M 组变化明显(P<0.05);Pearson 相关性分析显示,CD147 与 TNF-α、IL-6 及破骨细胞数量呈显著正相关,与 IL-10、TGF-β 呈显著负相关,破骨细胞数量与 TNF-α、IL-6 呈显著正相关,与 IL-10、TGF-β 呈显著负相关.结论:巴戟天丸能有效促进骨质疏松症骨折大鼠的骨愈合,其作用机制可能是下调CD147 的表达调节巨噬细胞活性,从而抑制破骨细胞过度活化和分化,最终减缓骨吸收,促进骨修复.
Objective:To clarify the molecular mechanism by which Bajitian Pill intervenes in osteopo-rotic fractures,and to investigate the role of CD147-mediated macrophage regulation of osteoclast function in fracture repair.Methods:Fifty SPF-grade female SD rats were selected and randomly divided into the normal(N)group,the model(C)group,the low-dose Bajitian Pill(L)group,the medium-dose Bajitian Pill(M)group,and the high-dose Bajitian Pill(H)group,with 10 rats in each group.Group N was not subjec-ted to modeling,while the other four groups were ovariectomized and raised for 12 weeks to establish a bone osteoporosis model.Then,a mid-section fracture of the right femur was made and fixed with an intramedullary needle.The osteoporosis fracture modeling was completed.After successful modeling,group L,M,and H were given Bajitian Pill at concentrations of 0.4,0.8,and 1.2g/mL by gavage once a day for 8 consecutive weeks.Group N and group C were gavaged with the same volume of normal saline at the same time.The bone density and bone biomechanical properties of the rats were examined.Bone tissues were taken for HE staining and TRAP staining to detect the generation of osteoclasts.Serum macrophage-related factor levels were detec-ted by enzyme-linked immunosorbent assay,and the protein expressions of CD147,RANKL,and OPG in the bone tissues were detected by immunoblotting.Results:Compared with group N,the bone density,maximum load,elastic modulus,and fracture energy in group C decreased(P<0.05).Compared with group C,the maximum load,elastic modulus,and fracture energy in group L,M,and H increased(P<0.05),and the in-crease in group M was more significant than that in group L(P<0.05),and the increase in group H was more significant than that in group M(P<0.05).The bone trabeculae in group N were arranged regularly and densely,with a complete structure and uniform bone matrix,without obvious gaps.In group C,the bone cal-lus tissue had sparse and disordered bone trabeculae.The pathological morphology of bone tissue in group L,M,and H improved significantly compared with group C.Compared with group N,the number of osteoclasts in group C increased(P<0.05),and the number of osteoclasts in group L,M,and H decreased(P<0.05),and the decrease was more significant in group M than in group L(P<0.05),and in group H than in group M(P<0.05).Compared with group N,the levels of serum TNF-α and IL-6 in group C increased(P<0.05),and the levels of IL-10 and TGF-β decreased(P<0.05).Compared with group C,the levels of TNF-α and IL-6 in group L,M,and H decreased(P<0.05),and the levels of IL-10 and TGF-β increased(P<0.05),and the changes were more significant in group M than in group L(P<0.05),and in group H than in group M(P<0.05).Compared with group N,the protein expressions of CD147 and RANKL in group C in-creased(P<0.05),and the protein expression of OPG decreased(P<0.05).Compared with group C,the protein expressions of CD147 and RANKL in group L,M,and H decreased(P<0.05),and the protein ex-pression of OPG increased(P<0.05),and the changes were more significant in group M than in group L(P<0.05),and in group H than in group M(P<0.05).Pearson correlation analysis showed that CD147 was significantly positively correlated with TNF-α,IL-6,and the number of osteoclasts,and significantly nega-tively correlated with IL-10 and TGF-β.The number of osteoclasts was significantly positively correlated with TNF-α and IL-6,and significantly negatively correlated with IL-10 and TGF-β.Conclusions:Bajitian Pills can effectively promote bone healing in osteoporotic fracture rats.The mechanism of action may be to down-regulate the expression of CD147 to regulate the activity of macrophages,thereby inhibiting the exces-sive activation and differentiation of osteoclasts,ultimately slowing down bone resorption and promoting bone repair.
