溴结构域蛋白4(Bromodomain-containing protein 4,BRD4)是BET蛋白家族的重要成员,其主要功能是识别并结合组蛋白上的乙酰化赖氨酸,作为表观遗传阅读器在调控基因转录中发挥核心作用.近年研究发现,BRD4在肾纤维化、肾细胞癌、狼疮性肾炎及糖尿病肾脏病等多种肾脏疾病的发病机制中扮演了关键角色.目前该领域的研究已从机制探索进入靶向干预阶段,多项实验证据表明,BRD4抑制剂能有效减轻肾脏病理损伤并改善肾功能.本文阐述了BRD4的分子结构与调控机制,重点总结了其在不同类型肾脏疾病中的致病作用及BRD4抑制剂的治疗潜力,旨在揭示BRD4作为肾脏疾病精准治疗分子靶点的潜力,并展望其临床应用前景.

Bromodomain-containing protein 4(BRD4)is a key member of the BET protein family.Its primary function is to recognize and bind to acetylated lysine residues on histones,acting as an epigenetic"reader"that plays a central role in transcriptional regulation.Recent studies have revealed that BRD4 plays a critical role in the pathogenesis of various kidney diseases,including renal fibrosis,renal cell carcinoma,lupus nephritis,and diabetic kidney disease.Currently,research in this field has transitioned from mechanistic exploration to targeted intervention,with multiple lines of experimental evidence indicating that BRD4 inhibitors can effectively alleviate renal pathological injury and improve kidney function.This review elaborates on the molecular structure and regulatory mechanisms of BRD4,summarizes its pathogenic roles in different types of kidney diseases and the therapeutic potential of BRD4 inhibitors,aiming to highlight the clinical prospects of BRD4 as a molecular target for the precision treatment of renal disorders.
