目的 探讨血细胞参数联合缺氧诱导因子-1α(HIF-1α)、唾液酸(SA)及乳酸脱氢酶(LDH)对骨髓增生异常综合征(myelodysplastic syndrome,MDS)病情发展的评估价值.方法 研究选取2021年7月—2024年12月江苏省中医院连云港医院收治的MDS患者90例作为MDS组,并分为低中危组(58例)与高危组(32例),另选50例健康者作为对照组.检测指标包括红细胞计数(RBC)、红细胞比容(Hct)、血小板计数(PLT)、白细胞计数(WBC)以及HIF-1α、SA和LDH水平.结果 MDS组PLT、RBC、Hct、WBC均低于对照组,且HIF-1α、SA、LDH均显著升高(P<0.05).高危组血细胞参数低于低中危组,血清指标则更高(P<0.05).相关性分析表明,MDS病情与血细胞参数呈负相关,与血清指标呈正相关(P<0.05).Logistic回归分析提示,血细胞参数为独立保护因素,血清指标为危险因素(P<0.05).ROC曲线显示,联合检测预测病情发展的灵敏度为86.79%,特异度为84.35%,AUC达0.814,优于单一指标(P<0.05).结论 血细胞参数联合HIF-1α、SA和LDH可用于MDS鉴别及病情评估,联合检测具有较高的预测效能.

Objective To investigate the value of hematologic parameters combined with HIF-1α,SA,and LDH in assessing the progression of myelodysplastic syndrome(MDS).Methods This study included 90 MDS patients admitted between July 2021 and December 2024 as the MDS group,further categorized into low-intermediate-risk(n=58)group and high-risk(n=32)group.Another 50 healthy subjects were selected as the control group.The detection indicators included red blood cell count(RBC),hematocrit(Hct),platelet count(PLT),white blood cell count(WBC),as well as the levels of HIF-1α,SA and LDH.Results In the MDS group,PLT,RBC,Hct and WBC were all lower than those in the control group,while the levels of HIF-1α,SA and LDH were significantly increased(P<0.05).Blood cell parameters in the high-risk group were lower than those in the low-intermediate-risk groups,whereas serum indexes were higher(P<0.05).Correlation analysis revealed that the severity of MDS was negatively correlated with blood cell parameters and positively correlated with serum indicators(P<0.05).Logistic regression analysis indicated that blood cell parameters were independent protective factors,and serum indexes were risk factors(P<0.05).ROC curve analysis showed that the combined detection achieved a sensitivity of 86.79%,a specificity of 84.35%and an AUC value of 0.814 in predicting disease progression,which was superior to single index detection(P<0.05).Conclusion The combination of hematologic parameters with HIF-1α,SA,and LDH could be used for MDS diagnosis and progression assessment,with the combined model showing superior predictive performance.
