肾小管酸中毒继发性骨质疏松症的诊疗新进展OA
Mechanisms,diagnosis,and treatment of renal tubular acidosis-associated secondary osteoporosis
肾小管酸中毒(RTA)是一种以肾脏酸化功能障碍为特征的临床综合征,其导致的慢性代谢性酸中毒是诱发和加重骨质疏松症的重要危险因素.目前,关于RTA继发性骨质疏松症的病理生理机制尚未完全阐明,其临床诊疗策略亦需进一步优化.本文旨在系统阐述各型RTA导致骨质疏松症的病理生理机制,包括慢性酸中毒对骨矿物质稳态、成骨与破骨细胞功能的复杂影响,并总结其临床特征.同时,本文探讨了针对RTA本身的碱剂补充治疗、纠正电解质紊乱、糖皮质激素和免疫抑制剂的应用,对骨质疏松症的影响.旨在为临床识别和管理这一特殊类型的代谢性骨病提供重要的理论依据和实践指导.
Renal tubular acidosis(RTA)is a clinical syndrome characterized by impaired renal acidification function,and the resulting chronic metabolic acidosis is an important risk factor for the development and progression of os-teoporosis.At present,the pathophysiological mechanisms underlying RTA-associated secondary osteoporosis have not been fully elucidated,and its clinical diagnostic and therapeutic strategies still require further optimization.This review systematically summarizes the pathophysiological mechanisms by which different types of RTA contribute to osteoporosis,including the complex effects of chronic metabolic acidosis on bone mineral homeostasis,osteoblast and osteoclast func-tion,and skeletal metabolism.The clinical characteristics of RTA-associated osteoporosis are also discussed.Further-more,this article reviews current therapeutic strategies targeting RTA itself,including alkali supplementation therapy,cor-rection of electrolyte disturbances,and the application of glucocorticoids and immunosuppressive agents,as well as their potential effects on osteoporosis.This review aims to provide an important theoretical basis and practical guidance for the clinical recognition and management of this special type of metabolic bone disease.
