目的 评估司普奇拜单抗在不同炎症水平慢性鼻窦炎伴鼻息肉(CRSwNP)患者中的疗效.方法 本研究为多中心、随机、双盲、安慰剂平行对照的3期CROWNS-2研究的事后分析.基于外周血及组织嗜酸性粒细胞、血清总免疫球蛋白E、血清胸腺和活化调节趋化因子、血浆嗜酸性粒细胞趋化因子-3等生物标志物基线中位数水平进行分层,评估司普奇拜单抗在不同标志物亚组中的疗效.结果 在各生物标志物基线中位数分层的亚组中,治疗24周后,司普奇拜单抗相比安慰剂可显著降低鼻息肉评分(组间最小二乘均值差值P均<0.000 1).此外,司普奇拜单抗在鼻堵、嗅觉减退、宾夕法尼亚大学嗅觉识别测试评分、鼻部症状综合评分等症状相关指标,以及Lund-Mackay CT评分及生活质量评分的改善程度亦显著优于安慰剂(组间最小二乘均值差值P均<0.05).在起效时间方面,药物在高炎症标志物亚组中起效更早,低标志物亚组虽起效稍晚但仍能实现显著获益.结论 司普奇拜单抗可为不同炎症水平的CRSwNP患者带来获益.其起效时间与炎症水平相关,提示可根据生物标志物表达水平指导个体化治疗,为CRSwNP的精准治疗提供依据.

OBJECTIVE Aimed to evaluate the efficacy of stapokibart in patients with CRSwNP stratified by baseline inflammatory biomarker levels.METHODS This post hoc analysis was based on data from the multicenter,randomized,double-blind,placebo-controlled phase 3 CROWNS-2 study.Patients were stratified according to baseline biomarker levels,including peripheral blood and tissue eosinophil counts,serum total immunoglobulin E,serum thymus and activation-regulated chemokine,and plasma eotaxin-3 levels.The efficacy of stapokibart was assessed across different biomarker-defined subgroups.RESULTS In subgroups stratified by the median baseline level of each biomarker,stapokibart significantly reduced nasal polyp scores compared with placebo after 24 weeks of treatment[all P values(vs.placebo)<0.0001].In addition,stapokibart significantly improved symptom-related outcomes,including nasal congestion,loss of smell,University of Pennsylvania smell identification test scores,and total nasal symptom scores,as well as Lund-Mackay CT scores and quality-of-life scores,compared with placebo across all biomarker subgroups[all P values(vs.placebo)<0.05].In terms of the onset of action,clinical responses were observed earlier in patients with higher baseline biomarker levels,whereas patients with lower baseline levels experienced a relatively delayed onset but still achieved significant clinical benefits.CONCLUSION Stapokibart provides significant clinical benefits for patients with CRSwNP across different biomarker levels.The onset of action is associated with biomarker levels,suggesting that biomarkers may help guide individualized treatment.
