艾灸与双氯芬酸钠凝胶治疗膝骨关节炎的即时效应和后效应:一项双盲双模拟随机对照试验OA
Immediate and Sustained Effects of Moxibustion Versus Diclofenac Sodium Gel for Knee Osteoarthritis:A Double-Blind,Double-Dummy Randomized Controlled Trial
目的:评价艾灸与双氯芬酸钠凝胶(Diclofenac Sodium Gel,DSG)治疗膝骨关节炎(Knee Osteo Arthritis,KOA)的即时效应与后效应,为艾灸治疗KOA提供部分循证依据.方法:采用多中心、双盲双模拟、随机对照试验设计,纳入139例40-75岁KOA患者,1∶1随机分为艾灸治疗组与DSG对照组,治疗4周并随访8周;艾灸治疗组:真实艾灸+安慰凝胶外用;DSG对照组:安慰艾灸+真实双氯芬酸钠凝胶外用.以西安大略和麦马斯特大学骨性关节炎指数(Western Ontario and McMaster Universities Osteoarthritis Index,WOM-AC)总分为主要结局指标,以视觉模拟评分量表(Visual Analog Scale,VAS),患者总体疗效评价(Patient Global Assessment,PGA)为次要指标,比较两组疗效、有效率与安全性.治疗参数:艾灸取犊鼻、内膝眼、足三里,每穴每壮燃烧30min,每周3次、隔日1次;凝胶每次每患膝涂抹4 g,每日3次.结果:治疗2周、4周时两组WOMAC、VAS评分均显著下降,两组间比较无统计学意义(P>0.05),短期即时疗效相当;随访8周时两组评分持续降低,DSG对照组WOMAC量化指标优于艾灸组(P<0.05);12周随访时艾灸组PGA显著改善占比数值高于对照组,但组间无统计学差异(P>0.05);两组仅出现轻微局部不良反应,安全性良好.结论:艾灸与DSG治疗KOA均安全有效,艾灸短期即时疗效与DSG相近,远期患者主观整体评价具有潜在优势,效应衰减更平缓、后效应更稳定,可作为KOA长期非药物优选管理方案.
Objective:To evaluate the immediate and sustained effects of moxibustion versus diclofenac sodium gel(DSG)in the treatment of knee osteoarthritis(KOA),and to provide evidence-based support for moxibustion in KOA management.Methods:A multicenter,doub-le-blind,double-dummy,randomized controlled trial was conducted.A total of 139 KOA patients aged 40-75 years were randomly assigned 1∶1 to the moxibustion group or the DSG control group.All patients received 4 weeks of intervention and 8 weeks of follow-up.Moxibustion group:real moxibustion plus a placebo gel for external use;DSG control group:sham moxibustion plus active diclofenac sodium gel for exter-nal use.The primary outcome was the total score of the Western Ontario and McMaster Universities Osteoarthritis Index(WOMAC).The secondary outcomes included the Visual Analog Scale(VAS)and Patient Global Assessment(PGA).The efficacy,response rate,and safety were compared between the two groups.Treatment parameters were set as follows:Moxibustion was applied to Dubi(ST35),Neixiyan(EX-LE5),and Zusanli(ST36),with each moxa cone burning for 30 minutes per acupoint,administered three times a week on alternate days.A total of 4 g gel was applied topically to each affected knee three times daily.Results:At 2 and 4 weeks of treatment,the WOMAC and VAS scores of both groups decreased significantly,with no statistically significant differences between groups(P>0.05),indicating comparable short-term immediate efficacy.At the 8-week follow-up,scores in both groups continued to decline;the DSG control group achieved better WOMAC scores than the moxibustion group(P<0.05).At the 12-week follow-up,the proportion of patients with marked improvement in PGA was higher in the moxibustion group,though the difference between groups was not statistically significant(P>0.05).Only mild local adverse reactions occurred in both groups,indicating good safety.Conclusion:Both moxibustion and DSG are safe and effective for KOA.Moxibustion yields short-term immediate efficacy comparable to DSG.It shows potential advantages in long-term patient global assessment with milder efficacy decline and more sustained late effects,and can be recommended as a preferred long-term non-pharmacological interven-tion for KOA.
